The Research Of New Rural Cooperative Medical Care System

After the failure of traditional rural cooperative medical care system and deterioration of the rural medical health, it is widely existed that peasants became economically poor or back to poverty because of diseases. Given China is big agricultural country, whether the government properly solves the medical security problem or not will directly affect rural economic development and social harmony. Chinese administration proposed to build governmental organizations which guide and support peasants to voluntarily join in cooperative supply system of serious illness among individual, collectivity and government. The new system has been carried out since the pilot work in 2003. We aimed to finish the plan of the new rural cooperative medical system extending to the hundred million peasants in China and reduce the economic burden of care treatment for peasants. At present, the pilot work has been carried out in some areas and made initial progress, but still exist many problems in practice which seriously affect sustainable development. On the basis of the history of old rural cooperative medical care system and prior research both from China and other countries, this paper try to compare the difference. After the analyzing the history of new cooperative medical care system, we find the evidence of existing problems about the pilot of new system. We also find that the sustainable development of new rural cooperative care treatment need stable financing system, reasonable compensation system as well as perfect match system.The remainders of the paper are organized as followed.Chapter 1, introduction, illustrating the research background, purpose and importance of rural cooperative care system and analyzing the research situation, direction and focus in China and around the world.Chapter 2, basic theory analysis of rural cooperative medical care system. Medical service has the characteristics of uncertainty, information asymmetry and externality, which means that market cannot meet the condition of Pareto optimal. This part deliberately discuss the cooperative economy theory, fair distribution theory, theory of medical market failures, theory of disease risk as well as social insurance theory and lay a firm theoretical foundation of future research.Chapter 3, the development and institutional analysis of new rural cooperative medical care system. This part summarize development history of Chinese rural cooperative medical treatment and study failing reason as well as analyze and comment the present situation of rural medical treatment by comparing the difference between the old and new system.Chapter 4, the constraints and causes that affect rural cooperative medical care system. The reform of cooperative medical treatment is complex project, involving countries, medical institution, and medical individual. This part analyzes the behavior from each party and point out the disadvantage respectively. We conclude there are lack of government function; the problem of specific medical establishment and personnel; the disadvantages of new cooperative medical care system; difficulty of financing and high cost of cash management.Chapter 5, developing rural cooperative medical care system and concluding clue. We try to improve the rural cooperative medical system by drawing the lesson from other countries. This part is mainly to study primary method of medical system of developing countries and find their advantages and disadvantages to summarize the teaching and experience.Chapter 6, improvement of the solution of new rural cooperative medical treatment. This part present the solution of problem that we should strengthen the duty of government; foster the reform of health service by improving the level of personnel; improve financing system ,increase compensation fee, reduce the threshold charges of medical treatment; and perfect relating system. Using a set of solutions to improve new cooperative medical treatment, this systematic project will have a long and sustainable development.
