Gastric ulcer is a clinical common and frequently encountered disease, which is a multifactorial disorder. This disease incidence of the whole world is about ten percent, It accordings to chronic course, periodic episode and stomachache of rhythmicity which is three cardinal appearances, male more than female, and to some extent it relates to season. Recently, with the rapid rhythm of life, the disease occurring of gastric ulcer tends to low ages, the most of people whose trouble is acute abdomen causing by gastrorrhagia are youth. In another problem, the elderly patients are worried about the ulcer to recur whose coruses are too long, even more than ten to twenty years. owing to the maximal confinement on the food and living, It impacts on quality of life of patient seriously. So gastric ulcer is yet a hot spot of global digestion at present.Many kinds of etiology of gastric ulcer are early known by people. In earlier period, Shay proposed the doctrine of dynamic disequilibrium between the aggressive and defensive factors, and Hollander drew off the doctrine of double barriers, both of them emphasize that the defensive factors are main factors which attenuate leading to gastric mucosa damaged, and great contribution to gastric ulcer. These theories provide the emphatic directions for modern clinical treatment. At same time, Chinese traditional medicine deems that the function of spleen depends on spleen-YANG which can educe risen,motive and divergent function. Spleen is earth-YIN which yang is always insufficient. It emphasizes the effect of yin-yang disequilibrium in the spleen and stomach disease. For example,《chapter of nervure syndrome treating with abdomen full and cold hernia and dyspepsia diseases》said:"heart and thorax cavity got great cold pain, the person vomit so that he is unable to drink and eat, pathogenic cold in abdomen,……"You Zaijing said in《Jinkui Yaolue Xindian》:"if we desire yin-yang harmony, we should seek middle-warmer energy so that we use Jan Zhong."To sum up, whether modern medicine or traditional medicine accentuate the important position of the defensive factors in onset and recurrence of gastric ulcer.Our department professors advanced compound preparation of WWY going through the long-term clinical practice. This broth has certain curative effect on treating the patients who get stomachache and relief with pressure which become serious to catch cold and to be tired, who get pain of empty stomach but relieve after eating, who get hydroptysis, anepithymia or limbs chilly, meanwhile, loose stool. To most of them, the symptoms decreased, the ulcers improved well, the quality of life elevated obviously. Recently, this broth has insufficient experiment evidences. So we chose to make animal model to research some kinds of cell defensive factors levels according to etiopathogenesis of insufficiency of spleen-YANG and gastric ulcer, and we have to approach the effects of WWY by resisting cellular oxidation, immunoloregulation and tissue repair in gastric ulcer of insufficiency of spleen-YANG.We chose the healthy ripe rats to be research objects in this experiment, which were divided to five groups at random, those are the common control group, the model control group, WWY high-dose group, WWY low-dose group, the ranitidine group. We introduced bitter cold rhubarb leading to decanta, gave 100% Rhei Decoction. We judge the information of model-made by utilizing cold water test to observe cold-resistant capability of the rats and observing the common state of these models such as vitality, daily activities, color pattern and luster, sedes shaping. We found that these rats'swimming time are shorter gradually, their cold-resistent capability descend, at the same time ,they turn to be loose stool, anorexia, abdominal distention, extenuation, archos, anus flare, arcus, action retardation, the frequency of activity drawdown, myasthenia of limbs, dispirited, piloerection, thin and loose clothing hair, less burnish, weight to lower. To compare with the common control group, the states of others were worse, it is proved that the models of insufficiency of spleen-YANG had made successfully. The eighth day, nothing by mouth but driking. The nighth day, we made the model of gastric ulcer by painting acetic acid from Okabe, and judged successfully by the pale in-congestion and dropsy aroud the ulcer.The tenth day after making models, according to surface area of human and rat to convert dosis, we used intragastric administration. Gave the model control group with 20ml/Kg/d the same volume of isotonic Na chloride; the WWY high-dose and low-dose groups with the quadruple,sigle clinical recommended dose of WWY concentrated solution, and we chose the ranitidine to be the positive control medicine in 27mg/kgbw/d dose. All of the groups are given twice a day continued 42 days. The 53th day, collected 3 ml blood from fossa orbitalis AtV every rat, segregated blood serum, put it into low temperature to conserve. After that, executed fastly by decapitation to get stomach, determined the length and width of ulcer. Cut a section of stomach tissue around ulcer near lesser curvature of stomach, after weigh, made tissue homogenate with absolute alcohol and normal saline by grind, and put supermatant into low temperature to conserve.Our aim to determine SOD,MDA,EGF from blood serum and 6-keto-PGF1αfrom gastric tissue, meanwhile, evaluate UI and UIR is to approach the effect of WWY on the defensive factors of gastric ulcer of insufficiency of spleen-YANG.SOD is a important antioxidase in vivo, which is to reflect organism capability of clear the oxygen free radical indirectly; MDA is a lipid peroxidation which is to reflect directly damaged extent of organism by aggressive of the oxygen free radical. It is manifest that accumulation of the oxygen free radical aggravates the ulcer acutely by the lower SOD and the higher MDA. 6-keto-PGF1αis the production of PGI2, Which is the highest level and strongest biological activity but short demiperiod of PGS. It generates from COX-2 of vascular endothelial cell. It can inhibit leukocytic activation of the damaged stomach tissue; contribute to immunological regulation; resist platelet aggregation and blood vessel extension; improve gastric mucosal blood flow. EGF is a sort of polypeptide hormones which can inhibit indirectly gastric acid secretion to protect cells; promote the synthnesis and externalization of blenn-glucoprotein of gastric mucosa and bicarbonate; increase gastric mucosal blood flow, activate cell migration of defective mucosa and promote cleavage and hyperhladia of vessels and tissues cells to heal ulcer which the most research in and is the most valuable to clinic. In this experiment, we ultilized classification of Okabe UI to evaluate UI and UIR except the common control group, and take fluor photometry to determine SOD,MDA activities of blood serum, and nonequilibrium radio-immunity to determine 6-keto-PGF1αof gastric tissue and EGF of blood serum. We observe and compare the weight and pathological change of rats btween before and after making models and treatment.This experimental result manifest conclusion below. The weight of rats lessen obviously contrasting before making models. After treatment, the weight increase gradually, it is lower amplitude in the model control group, but highest amplitude and UIR in WWY high-dose group. Contrast to the model control group, it can elevate the activities of SOD,the levels of 6-keto-PGF1αand EGF, depress the content of MDA at same time in treatment groups, of which WWY high-dose group is the most obvious near the common control group, which demonstrate resistive effect on cellular oxidation by oxygen free radical and relief on the damaged mucosa. It can priming immune regulation, strengthen reepithelialization and promote ulcer tissue repair. There are the best improvement in pathological section of WWY high-dose group, most of which had healed.To sum up, we analyze the mechanism of action for WWY treating gastric ulcer of insufficiency of spleen-YANG in the ascept of gastric mucosal defensive factors. The therapeutic efficacy of WWY relates to the increase in antioxidase of rat blood, growth factor family and prostacylin of gastric tissue. This is prompt rapeutic action of WWY that is:①adjust disequilibrium of aggressive factors and defensive factors by resisting cellular oxidation.②regulate organism immune system, strogthen the capability of eliminating pathogen.③promote the healing of ulcer tissue to realize the repair of focal disease. Contrast to early restrain acid medicine, WWY can enhance gastric mucosal defensive ability of gastric ulcer of insufficiency of spleen-YANG, adjust disordered digestive function, raise the ulcerative curing rate. This provides us emphatic experiment evidences to traditional medicine clinical therapy for gastric ulcer in differentiation of symptoms and signs. Simultaneously, it establishs the foundation of subsequent research on defensive factors'DNA,mRNA levels about gastric ulcer of insufficiency of spleen-YANG for WWY.
