PrefaceSquamous cell carcinoma is a malignant neoplasm which origin from keratinocyte which located at epidermis and skin adnexa.The tumor cell is a kind of unmature keratinocyte.Psoriasis is a common,chronic,recurrent,inflammatory disease of skin. Recently researches shows that the hyperplasy of keratinocyte and the infiltration of lymphocyte and neutrophilic leukocyte induced this disease.CD147,which is also called EMMPRIN(extracellular matrix metalprotein), Basigin(basic immunoglobulin)or M6,Hab/18G.It is a member of the immunoglobulin superfamily and is a widely expressed membrane protein.CD147 was found strengthen expressing in many kinds of tumors,and related to the infiltration and metastasis of these tumors.The newly studies indicated that the expression of CD147 were inverse correlation with the differentiation of kerationocytes.This phenomenon may infer that CD147 might inhibit the differentiation of karationocytes.Recent therapeutic investigation found that it can be the therapeutic targets of many kinds of diseases, further therapeutic research about CD147 for the particular working mechanisms is valuable.The composition of T lymphocyte subset in the lesions of dermatonosis demonstrated the topical immune state.The study of the regulation of it conduce to research the pathogenesy,evaluation of pathogenetic condition and the therapeutic measures.We examined the expression of CD147 in the lesions from patients with cutaneous squamous cell carcinoma(SCC)and psoriasis vulgaris to investigate the role of CD147 in the differentiation and proliferation of keratinocytes.Associated with the T lymphocyte subset of the squamous cell carcinoma(SCC)and psoriasis vulgaris,we can infer its possible effect on the local immune state in SCC and psoriasis.This study try to explore a potential new way to cure SCC,psoriasis and other dermatosis due to keratinocyte abnormal proliferation,and provide some new idea to the etiology and pathogenesis,even the treatment.Materials and MethodsWe choose 50 cases of SCC,28 cases of psoriasis vulgaris and 25 cases of normal skin as control,and all is diagnosed by clinical and pathological measurement by at least two physicians.Applying immunohistochemistry technique to analyze CD147 expressions and distribution in the tumor cells of squamous cell carcinoma(SCC), keratinocytes of psoriasis vulgaris and keratinocytes of normal skin specimen.By the way,we examined the T lymphocyte subgroup infiltration and distribution which are marked with CD4 and CD8.Assessment of the result:Observing the magnitude of the stain of lesions of SCC and psoriasis vulgaris under optical microscope.Count 300 cells randomly from each specimen,then record the magnitude of stains as 0~3 scores,and record the persent of positive cells,positive cells has four scores:<5%is 0 score,5%to 25%is 1 score;25%to 50%is 2 socre;more than 50%is 3 score.And multiply these two scores to obtain the IRIDI score of the sample. The determination of the infiltrating T lymphocyte subset:5observe the stain of SCC and psoriasis vulgaris under optical microscope,count T cell of 5 noncontinuous high power visual field.Statistical analysisSoftware SPSS13.0 was used and the data was analysed by analysis of variance. Statistical significance was established if the two-sided P value of a test was less than 0.05.ResultThe expression of CD147:CD147 expressing in all of the specimen.The average IRIDI value of SCC tumor cells is 6.66±0.53,higher than the keratinocytes of psoriasis(5.50±0.89)and normal control group(5.62±0.61),and there is statistical significance(P<0.05).The average IRIDI value of SCC tumor cells,keratinocytes of psoriasis and keratinocytes of SCC,are all higher than the normal control group (1.80±0.51).The difference has statistical significance(P<0.05).CD147 expressed strongly in the infiltrating cells in SCC and psoriasis.The average IRIDI value of SCC (5.98±0.54)has no statistical significance with that of psoriasis(6.18±0.80)(P>0.05). T lymphocyte subgroup:The average CD3~+ T lymphocyte in the epidermis of SCC is 45.8±14.7,and the average CD3~+ T lymphocyte in the epidermis of psoriasis is 112.7±32.7,with statistical significance(P<0.05).The CD4 and CD8 ratio in SCC is 1.5:1,and the ratio in psoriasis is 1:1.6.There is statistical significance(P<0.05).The CD4 and CD8 ratio in the epidermis of psoriasis(1:1.6)and that of dermis (1.1:1)has statistical significance(P<0.05).Conclusion1.CD147 expression in keratinocytes of SCC and psoriasis vulgrais was stronger than that of keratinocytes of normal skin with statistical significance(P<0.05).But CD147 expression in keratinocytes between SCC and psoriasis vulgrais without statistical significance(P>0.05).2.CD147 expression in tumor cells was stronger than that in keratinocytes of SCC and psoriasis vulgrais with statistical significance(P<0.05).3.CD147 strongly expressed in the infiltrating T lymphocyte in both SCC and psoriasis vulgrais without statistical significance(P>0.05).4.The ratio of CD8 positive T lymphocyte is higher than the normal control. CD147 involve in the pathogenesis by activating the T lymphocyte.
