Objective:To investigate the expression of nuclear factor- kappaB (NF-κB)and vascular endothelial growth factor(VEGF)correlate with clinicopathological stage and angiogenesis in primary renal cell carcinoma(RCC).Methods:Immunohistochemistry was used to detect the expression of NF-κB p65 and VEGF in 40 cases of RCC and 10 cases of renal tissue. Microvessel density(MVD)was assessed by immunohistochemical staining for endothelial cells using anti-CD34 monoclonal antibody.RCC cases were divided into early staging group(Robson stagingⅠ,Ⅱ)and advanced group(Robson stagingⅢ,Ⅳ).Chisq test,student t test, Spearman rank correlation test were used for data statistic analysis.Result:(1)26 of 40 RCC were positive for NF-κB p65 protein with a 65.0%positive rate,but there was lower expression in normal renal tissue,Statistic significance existed between the two groups(P<0.01). The expression of NF-κB p65 were positive in 17 cases(85%)and negative in 3 cases(15%)of 20 cases of advanced RCC,were positive in 9 cases(45%)and negative in 11(55%)of 20 cases of early staging RCC, Statistic significance existed between the two groups(P<0.05).(2)The positive rate of VEGF in RCC(27/40;67.5%)was significantly higher than that in normal renal tissue(20.0%,P<0.05).The expression of VEGF were positive in 17 cases(85%)and negative in 3 cases(15%) of 20 cases of advanced RCC,were positive in 10 cases(50%)and negative in 10(50%)of 48 cases of early staging RCC,Statistic significance existed between the two groups(P<0.05).(3)MVD parameter was 63.87±19.51 in RCC and 43.58±3.11 in control group, Statistic significance existed between the two groups(P<0.05).MVD parameter was 78.16±16.84 in advanced group and 58.16±14.73 in early staging group,Statistic significance existed between the two groups (P<0.05).NF-κB negative staining tumor(55.44±4.72)showed a significant lower mean MVD per microscopic field than NF-κB positive tumor(77.08±15.59),Statistic significance existed between the two groups(P<0.01).The same result was found on VEGF(P<0.01).MVD parameter was 86.54±10.76 in both NF-κB and VEGF positive staining group,was 67.04±2.42 in either NF-κB or VEGF positive staining group, was 51.64±3.45 in neither NF-κB nor VEGF positive staining group. Statistic significance existed among the three groups(P<0.01).(4)22 of 40 RCC were both NF-κB p65 and VEGF positive,9 of 40 RCC were both NF-κB p65 and VEGF negative.The expression of NF-κB p65 has a positive correlation with that of VEGF in RCC(P<0.01,r=0.498).(5) NF-κB and VEGF expression is highly related with the prognosis of RCC patients.Conclusions:NF-κB and VEGF was activated abnormally in RCC, especially in advanced RCC.The expression of NF-κB and VEGF correlated with angiogenesis in RCC.NF-κB and VEGF may alter the biological behavior of RCC by prompting angiogenesis.NF-κB may positively adjust high VEGF expression in RCC.The expression of NF-κB and VEGF may reflect the differentiation level of RCC and is highly related with the prognosis of RCC patients.They may be a new target for curing of RCC and serve as key for researching mechanism of RCC.
