The Clinical Research On YI SHEN XIE ZHUO FANG Treating Renal Anemia In Chronicity Kidney Disease

ObjectiveTo summarize the distribution rule of traditional Chinese medicine syndrome in chronicity kidney disease(CKD)renal anemia(RA).To observe the clinical effect and safety on YI SHEN XIE ZHUO FANG(YSXZF)treating RA(renal anemia)in chronicity kidney disease.To demonstrate the feasibility and clinical significance of the "YI SHEN and XIE ZHUO" method on treating renal anemia in chronicity kidney disease(CKD).Methods93 cases patients who came from renal clinic or ward in ShenZhen Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine,They were divided into groups randomly in double-blind condition,included the treatment group(47 case) and control group(46 case);The intervention therapy were The YI SHEN XIE ZHUO FANG ()method in the treatment group,also it was The YI XUE SHENG collocystis in the control group.At the same time,Two group patients were given the food and drink of low salt,adequate quality low protein,low fosforus,low fat.The ingestion of protein was controlled in 1～0.8mg/kg.d^-1,The blood pressure of 93 case patients was controlled 120～140/70～90mmhg,and the other therapies were included,for example,the anti-infection treatment,the diuresis treatment,the anticoagulation treatment and the subdue sebum treatment.The periodicities of observation were four month,and it was analyzed in two therapy later.The during the treatment, All patience were demanded to examine the Blood Routine,the reticulocyte,kidney function,liver function and the correlated TCM symptoms.Also The adverse effecttion were monitored in all treatment cycle.Result1,The TCM syndromes could be found in the 93 cases as languor-syndrome,yin-deficiency, qi-deficiency,yang-deficiency,damp-heat and blood stasis syndrome.The highest(39%)was found in qi-yin deficiency syndrome type.The damp-heat syndrome and blood stasis syndrome were the most frequently encountered in the accompanying syndromes.2,In the therapeutic effect evaluation of the two groups.The total effective rate of the treatment group was 97.8%;That of the control group was 91.3%;there was distinctive difference between groups by statistical analysis(P＜0.05).3,The accumulative mark of TCM had distinctly decreased after treatment comparing with before treatment in every group(P＜0.05).In the comparisons of the TCM accumulative mark after treatment,there was no statistical different between the two groups in comparisons of the dizziness,the deficient breath and low talk,the deficiency spirit complexion,the extremities anaesthesion(P＞0.05),also there was distinctive statistical different between the two groups in comparisons of the lassitude hypodynamia,the lumbodynia,the brown urine and the poor appetite(P＜0.05).4,In the examination of laboratory,there was significantly different between the two groups in comparisons of The reticulocyte count,The hemoglobin count,The hematocrit,The akaryocyte count and The erythropoietin after four week treatment(P＜0.05).The levels of The reticulocyte count,The hemoglobin count,The hematocrit,The akaryocyte count and The erythropoietin in patients with the control group were significantly lower than those in patients with the treatment group(P＜0.05).5,In the examination of the renal function,there was significantly different between the two groups in comparisons of The glomerular filtration rate(GFR)and The serum urea nitrogen after one course of treatment(P＜0.05).The level of The glomerular filtration rate recovery in treatment group was significantly higher than that in the control group(P＜0.05).also the level of The serum urea nitrogen clearance rate was significantly variability(P＜0.05).6,In the observation of the drug adverse affections,one case of the sicchasia disgorging was discovered in the treatment group.Two cases of the blood pressure fluctuation were discovered in the control group.Also,There no obviously abnormality of the liver function,the fecal occult blood,the serum electrolyte in the cases.ConclusionThe viewpoints of the renal-deficiency's essence and the damp-heat blood-stasis's accompanied symptoms were the etiopathogenisis of TCM in chronicity kidney disease renal anemia.And it was proved that The YISHENXIEZHUO FANG had significant clinical effectiveness in midrange renal anemia.
