Objective: To evaluate the pros and cons of the two patterns in making the transplanted hepatoma models of rats, and to probe the therapeutic effect and mechanism of treatment of transplanted hepatoma with of Kang-Lai-Te ( KLT ) by interventional therapy.Methods: Forty SD rats were randomly divided into two groups (group A and group B, 20 rats in each group). Concentrated suspensions of Walker-256 tumor cells were percutaneous transhepatic injected into the livers of group A rats guided by ultrasound, and the same suspensions were injected in to the livers of group B under direct vision by laparotomy. Then observe and compare the successful rate, the survival time of the tumor-bearing rats, the development pattern of the transplanted hepatoma and the enhancement pattern of the liver tumor with contrast-enhanced ultrasonography (CEUS) between two groups.Thirty SD rats with transplanted hepatoma were randomly divided into three groups with 10 rats in each group. Different drugs were percutaneous transhepatic injected into the implanted hepatoma guided by ultrasound, one group with saline (Saline group); one with anhydrous ethanol (Ethanol group); and one with KLT (KLT group). Six days after transplantation, the medicines were injected into the tumors every 5 days. After being injected 3 times, the rats were killed. The weight of rats, the size and the internal echo of the tumor, the number of the tumor vessels, and the enhancement pattern of CEUS were detected. Compare the indexes of blood cells, liver and kidney function before modeling and before being killed in each group respectively. Rats were killed and the tumor tissues were examined by pathology. The expression of Ki-67 in the tumor cells were detected by immunohistochemistry and apoptotic cells were visualized by TUNEL assay.Results: The successful rate of group A was 75% and the tumor-bearing survival period was 26 days, while the successful rate of group B was 98% and the tumor-bearing survival period was 24 days. The enhancement pattern of transplanted hepatoma in rats is similar with the enhancement pattern of malignant tumor in human.The weight and the tumor growth index (TGI) of Ethanol group and KLT group were markedly higher than that of Saline group (P<0.05), but there was also statistically significance between the Ethanol group and the KLT group (P<0.05).When the tumor underwent examination of CEUS, KLT group still showed as the specific enhancement pattern of malignant tumor before being killed, and there was no significant difference in the time intensity curve when compared with the Saline group. There was no significant difference in the indexes of blood cells before modeling and before being killed. Comparing to before modeling, the liver and kidney function of Ethanol group was significantly different from that of before being killed, while the distinction was not remarkable in KLT group and Saline group. The differences of Ki-67 index and the tumor cells apoptosis index among the three groups were remarkable, and the apoptosis index of KLT group was significantly higher than that of Saline group and Ethanol group.Conclusion: The rat model making by laparotomy had a high successful rate, which is more suitable for the study of imaging diagnosis and interventional therapy. Six or seven days after transplanting are the best time for the treatment of intra-tumoral injection.This study shows that KLT has certain curative effects on transplanted hepatoma rats, but it is not as good as enthanol. However, KLT has no bone marrow suppression and side effect to liver and kidney function. Its mechanism to inhibit the tumor cells could be induce tumor cells apoptosis and inhibit tumor cells proliferation.
