Objective:To determine the plasmal level of calcitonin-gene-related peptide (CGRP)and neuropeptide Y(NPY)in patients with IBS;To evaluate the mental status such as anxiety and depressive state of the patients with IBS;and study the relationship between CGRP,NPY and mental factor.Methods:1,Based on RomeⅢcriteria,61 IBS patients from clinic service in xiangya hospital from 2007.6—9,including 34 diarrhea-predominant IBS (D-IBS),17constipation-predominant IBS(C-IBS),10mixed IBS (M-IBS),were choosed in this study.2,These 61 patients were evaluated with Self-rating Anxiety Scale (SAS)and Self-rating Depression Scale(SDS).There are 19 patients with IBS had mild anxiety,7 had midrange anxiety;16 had mild depressive,23 had midrange depressive,4 had severe depressive;11 had no anxiety or depressive state;22 had mild anxiety or depressive state;24 had midrange anxiety or depressive state;4 had severe anxiety or depressive state.3,The plasmal level of CGRP and NPY were measured by radioimmunoassay(RIA).Results:1,The plasmal level of CGRP was significantly higher in IBS patients than it in the normal controls(P<0.05),and there were no significant difference among these three types(IBS with diarrhea,IBS with constipation and mixed IBS)。Whereas it was significantly higher in IBS patients with abdominal-pain than without,as to NPY level there was no significant difference between them.2,The plasma NPY level in IBS patients was significantly lower than it in the normal controls(P<0.05),and there were no significant difference among these three types.Whereas it was significantly lower in IBS patients whose SDS or SAS scored unnormal, but there was no significant difference between the two-unnormal stages,as to CGRP level there was no significant difference among these different anxiety/ depression state.3,The SAS scores average of 61 IBS patients were 48.31±9.32, SAS scores average were 56.49±8.35;the average scores of SDS in the group of young and midlife people were significantly higher than it in the group of agedness(P<0.05),but the average scores of SAS were no significant difference in different groups. The scores of SDS were no significant difference among the patients with different culture level and sex(P>0.05);but the scores of SAS were higher in persons of lower culture level.Conclusions:1,The plasmal level of CGRP was significantly higher in IBS patients than it in normal person, the plasma level of NPY in IBS patients was significantly lower than it in normal person and there were no significant difference among these three types(IBS with diarrhea,IBS with constipation and mixed IBS)2,The plasmal level of CGRP was significantly higher in IBS patients with abdominal-pain than without(P<0.05):which pompted that the CGRP was concerned with viscera hypersensitive and result in abdominal pain in IBS patients.3,The plasma level of NPY was significantly lower in IBS patients whose SDS or SAS scored higher;which pompted that the NPY was concerned with the depressive and anxiety state in IBS patients.4,The attack rate of anxiety or depressive in patient with IBS was 81.97%.5,The IBS patients in midlife and young were easier to depressive state than older. The IBS patients of lower culture level were easier to anxiety state than persons of higher culture level.
