Objective:Observe the clinical efficacy of acupuncture treatment of constipation-predominant irritable bowel syndrome and its impact on the quality of life of patients. By changes in the detection of patients before and after treatment in plasma5-HT, CGRP and NPY level, analyse the influence of acupuncture treatment of IBS-C patients’ brain-intestinal peptide level in the body and try to explore the acupuncture treatment mechanism of IBS-C from the perspective of brain-gut axis.Method:By simple random number table,60patients with IBS-C were randomly divided into two groups, the acupuncture treatment group30cases, the control group of30patients to oral lactulose oral solution. Observe the improvement of clinical symptoms and quality of life of two groups of patients before and after treatment and follow-up(two months after the end of treatment). Using ELISA assay to detect the plasma5-HT, NPY and CGRP level of30healthy volunteers and two groups of patients with IBS-C before and after treatment.Result:(1) In terms of clinical improvement, comparing the two groups at baseline before treatment were comparable (P>0.05), acupuncture treatment can significantly improve the clinical symptoms in patients with IBS-C, and the recent and forward curative effect was superior to the control group(P<0.01).(2) Both groups can significantly improve the quality of life of patients (P<0.01), the improvement of long-term quality of life of patients in treatment group was superior to the control group(P<0.01).The four factors including dysphoria(Q1),interference with activity(Q2),health worry(Q4) and food avoidance(Q5) in the patients quality of life scale improve obviously, the treatment group is superior to control group, with significant difference(P<0.05).(3) Overall curative effect of treatment group is better than that of control group (P<0.01).(4) The levels of5-HT, NPY and CGRP in the two groups of patients with IBS-C were significantly higher than those in healthy volunteers, the difference was statistically significant (P<0.01).(5) After treatment, both groups can make the patients’plasma5-HT, NPY and CGRP levels drop, the differences of5-HT,NPY and CGRP levels were highly significant in the treatment group (P<0.01). While the differences of5-HT and NPY levels were highly significant in the control group (P<0.01), but the difference of CGRP level was no statistically significant (P>0.05).There were no significant differences of5-HT and NPY levels between the two groups after treatment (P>0.05), but the difference of CGRP was highly statistically significant (P<0.01).The downward trend of5-HT,NPY and CGRP level was better in the treatment group.Conclusion:(1) Acupuncture treatment can significantly improve the clinical symptoms in patients with IBS-C, and improve their quality of life, the overall efficacy is better than that of control group.(2) The5-HT, NPY and CGRP levels in patients with IBS-C compared with that in healthy volunteers were increased, and the patients are accompanied with varying degree of abdominal pain or discomfort, suggesting that the abnormal brain-intestinal peptide level was closely associated with the IBS syndrome.(3) Acupuncture treatment can make the patients’ plasma5-HT, NPY and CGRP levels drop, alleviate the degree of abdominal pain and discomfort, reduce visceral hypersensitivity in patients with intestinal dynamic disorder, so as to achieve the purpose of treatment of IBS-C.(4) Benign regulation of brain-intestinal peptide level in patients with IBS-C may be one of the effect mechanisms of acupuncture treatment of this disease.
