Following the development of the technique about transfusions and the general application of component blood transfusion, the phenomenon of the transfusion reaction (TR) and the ineffective transfusion emerges by degrees. And the issues of the transfusion reaction and the ineffective infusion of platelets have got universal reconstructions, but the issues of the ineffective infusion of red blood cells are still ignored, simultaneously, few articles were published.Objective: The present paper assess the influencing factor about the effective power of red blood cell transfusion in pediatric patient with hematopathy in order to improve the effect of blood transfusion and to serve as a guide for the clinical blood transfusion.Methods: 311 patients in the pediatrics of hematopathy in the west China second university hospital, from January 2006 to August 2007, were reviewed, and the rate of effective transfusion and its relationship to patients'sex, age, disease, fever, times of transfusions and volume of previous transfusions were analyzed. And meanwhile, the clinical effectives of red blood transfusion in 13 patients whose non-specificity antibodies were positive were reviewed.Results: Of the 563 times'transfusions (add up to 311 patients and to 955.5 units), effective transfusions occurred in 390 times (69.27%). The ratio of boy's red blood cell transfusions was 67.42%, and the ratio of girl's was 72.46. And the ratio of transfusions varied in different ages: 81.25% (<1 y), 68.79% (1~5y), 67.65% (5~10y) and 69.41% (>10y), and that also varied with different disease: 70.00% (aplastic anemia) and 70.94% (leukemia), varied in different times of transfusions: 75.24% (1 time), 67.61% (2~3 times) and 48.68% (>3 times), and varied with different volume of previous transfusion: 72.87% (<3U), 61.63% (3~6U) and 47.62% (>6U). And the ratio of transfusion in fever was 60.00%. Through statistics, we found statistical significances in the clinical effectives among the fever or not, and among the different times of transfusions and volume of previous transfusions, but not among the different sex, age, disease. And meanwhile, of 39 times'transfusions (non-specificity antibodies were positive), effective transfusions occurred in 31 times (74.49%).Conclusion: The effective transfusions were not related to patients'sex, age, disease, but were related to the fever, times of transfusions and volume of previous transfusions. And the much more times of transfusions, and the much more volume of previous transfusions, the much lower of the effective transfusion. And meanwhile, the effective transfusions of children whose non-specificity antibodies were positive,transfused the blood products without the related antigens of red blood cells, were not less than the mean(69.27%).
