The Study Based On The Relativity Between HVTT,HTT Measured By Contrast Enhanced Ultrasonography And The Grade Of The Hepatic Function In Hepatocirrhosis

Objective To investigate the methods of measuring hepatic artery transit time (HATT),hepatic vein transit time (HVTT) and hepatic artery to vein transit time (HTT) by real-time gray scale contrast enhanced ultrasonography (CEUS) and discuss the relativity between HVTT,HTT and the grade of the liver function in hepatocirrhosis.Method Sixty patients of hepatocirrhosis after HBV were divided into 3 groups on the basis of the liver function by Child-pugh method, 20 cases in grade A, B and C group respectively, Compared with 15 cases of healthy people as control group. The inner diameter of the right hepatic vein and the type of Doppler spectrum of the right hepatic vein were measured by ultrasonic equipment, then each cases was received the real-time gray scale CEUS. After all of the cases were injected the contrast agent SonoVue 1.6ml through the left elbow vein by intravenous bolus injection, the process of of SonoVue first appeared in the right hepatic artery,in the right hepatic veins was observed. HATT,HVTT was recorded, HTT was counted. After analyzed the results by statistical method,the value (p<0.05) was considered significance. Descriptive statistics were used to summarize the data. Bivariate correlations analysis was used to examine the relationship between HATT, HVTT, HTT and the level of hepatic function.Result Comparing with the control group, The inner diameter of the right hepatic vein of experimental group was decreased remarkably, the type of Doppler spectrum of the right hepatic vein of experimental groups was changed into abnormal wave shape, The HVTT and HTT of experimental groups were shortened obviously, but there was no difference in HATT between experimental groups and control group. Along with the aggravation of liver function, The inner diameter of right hepatic vein became smaller and smaller, and the type of frequency spectra of right hepatic vein turned into HV₂ from HV₁. HVTT and HTT were shortened significantly with the aggravation of liver function by Child-pugh grade method.Conclusion Measuring HVTT and HTT by the real-time gray scale Contrast-Enhanced Ultrasonography can reveal hemodynamic changes in hepatocirrhosis. Both of them can be applied in qualitative evaluation of Child-pugh grade,and HTT is more valuable than HVTT.
