Furcation perforation often leads to a very perplexing and frustrating problem in endodontic therapy, often have a doubtful prognosis .The prognosis of a perforation depends on the location and size of the perforation, the timing elapsed before the defect is repaired, the character of the material is one of the most critical factor in the impact. Conventional material such as calcium hydroxide, amalgam have been used for repairing this perforation. However, due to perforation is bottomless, material easily gets into the periodontal space, causing further damage of the periodontal tissue.Internal matrix technology is to prevent the filling materials into the periodontal space, put biocompatible material under filling material such as amalgam or galss ionomer cement. Compared with the traditional repair techniques, it have obvious advantages, on the one hand can stop bleeding, isolate leakage; On the other hand can provide support for the filling material and to prevent the overextusion. At the same time have antibacterial,anti-inflammatory effects. Many materials have been used as internal matrix, such as calcium sulfate, hydroxylapatite, calcium phosphate cement, collagen and so on. But everyone of them has disadvantage. Traditional Chinese drug have not been reported to used as internal matrix. Therefore, in this research, a Chinese gall compound recipe as internal matrix.This study used self-made barriers within the compound gallnut as materials, observe the restoration of the closed properties of materials and prevent the overextrusion the role of the pulp used in clinical treatment at the end of piercing1 The experiment of inhibiting bacteria of Chinese nutgall as internal matrix material in vitroPorphyromonas gingivalis(Pg) and Fusobacterium nucleatum (Fn)were common in periodontal and endodontic disease. An in vitro antibacterial activity (MIC) and continuing inhibitory effect of compound gallnut and Bletilla striata were tested in this study. The results showed that the two of the compound gallnut have the same effect and the MIC, 48h sterilization maximum rate of 38 percent, while the same concentration of Bletilla striata was no inhibitory effect, the first 24 h and the sterilization rate was only 6.3 percent. It suggest that at the same concentration, the inhibitory effect of compound gallnut was better than Bletilla striata. The results showed that all experimental groups demonstrated significant dye penetration. Group A had the great value of dye penetration and followed by group R,group AM,group MTA and group RM .Statistical analysis revealed significant differences at 95% between all tested groups(P<0.005),except group AM and group R,group AM and group MTA. There were no overextrusion in group AM and group RM. It suggest that Chinese gall compound recipe as internal matrix material to repair furcation perforation could reduce the microleakage, and could prevent overextrusion of repair material effectively.3 The animal study of experimental furcation perforation treated with Chinese nutgall as internal matrixA model of furcation perforation was made in dog'teeth and repaied with compound recipe, amalgam, MTA, and the blank control without any treatment. HE slice was made to evaluate the inflammatory. Effects were evaluated after 1 and 3 months. The results showed that gallnut group hadⅢ~Ⅳinflammation after 1 month, andⅡ~Ⅲafter 3 months. Parts of the material had been degraded; no hard tissue were observed. Group amalgam hadⅢ~Ⅳinflammation after 1 month, and still after 3 months. A mass of lymphocyte could been seen in the microscope. And no hard tissue either. Group MTA after 1 month, and most of inflammation had disappeared after 3 months,hard tissue could be observed. Blank control group hadⅢ~Ⅳinflammation after1 and 3 months. Granulation tissue could be seen in through the perforation, and it was full of inflammatory cells and blood capillary; no hard tissue were observed. It suggest that the compound gallnut as internal matrix could be degrade, and have anti-inflammatory effect.In a conclusion, the compound gallnut used to repair furcation perforations as Internal matrix could fulfill antibacterial, sealing, resorptable, atiinflammatory ability. Therefore, it can be considered to used in clinics.
