The TCM-based Clinical Research Of Type 2 Diabetic Macroangiopathy

OBJECT:To observe the differences and distribution of the fibrinogen and blood lipid in type 2 diabetic macroangiopathy TCM different type. Through an initial objective of TCM research for type2 diabetes with macrovascular,it will provide clinical basis for different type of Chinese TCM treatment.METHODS:150 patients with type2 diabetes macroangiopathy were brought into experimental group(aged from39 to 92; male 80,female 70).The experimental group was individed to four groups. 21 cases were served as type of extreme heat with YIN asthenia ,66 cases were served as type of deficiency of both QI and YIN, 28 cases were served as deficiency of both yin and yang, and others were served as Blood stasis. While 30 healthy people and 30 patients of type 2 diabetes without vacular lesion. To make use of magnetic bead method to fix the fibrinogen. To make use of automatic biochemistry analyzer to fix blood lipid and blood sugar.RESULT:1 .Among the different types of Chinese medicine with type2 diabetes macrovascular,both Qi and Yin deficiency was maximum,followed by blood stasis, the yin and yang deficiency, Yin Heat.2.The patients of type2 diabetes were much easier to cause the hyerfibrinogenemia and hyperlipidemia than the healthy. The patients of type2 diabetes macroangiopathy were more obviously than two contrast groups (P<0.01). Among all the TCM types,the fibrinogenemia and blood lipid of the type of extreme heat with YIN asthenia was the lowest, and compared to the other three types ,therse was a statistical significance(P<0.01). The fibrinogenemia and blood lipid of deficiency of both yin and yang and Blood stasis were higher than the two other types, but in the comparison of the two types there was no statistic significance. 3. There was obvious correlation with the TCM type of syndrome and course of disease and age . And compared to the other three TCM syndromes , the course of disease of the type of extreme heat with YIN asthenia was shortest;and age was smallest, and compared to the other three types ,therse was a statistical significance(P<0.01). the course of disease of deficiency of both yin and yang and Blood stasis was longest,and age of that was highest,but in the comparison of the type of deficiency of both yin and yang and type of Blood stasis,there was no statistic significance.4.The comparison of BMI and HbAlc in all TCM types showed there was no statistic significance.5.There was linear correlation with the fibrinogen and TG,TC and LDL (P<0.01).There was inverse correlation with the fibrinogen and HDL(P<0.01).There was no obervious correlation with the fibrinogen and FPG and HbAlc(P >0.05).CONCLUSION:TCM types of type2 diabetic macroangiopathy had the tendency which from type of extreme heat with YIN asthenia through type of deficiency of both QI and YIN to deficiency of both yin and yang and Blood stasis. With the development of the course of disease,the level of the the fibrinogenemia and blood lipid had the tendency to update. In all TCM types the type of deficiency of both QI and YIN were most. The fibrinogen and blood lipid may be regarded as the objective index of different degress of condition. There was obvious correlaton with the fibrinogen and blood lipid.
