Objectives:Understanding the allocation of manpower resources in Changchun community health service is to provide a basis for reasonable allocation of health service human resources; and supply a scientific basis for health administration and training plan and professional skills of academic background of health service personnel; offer reference for the relative governmental departments which are responsible for making policies on human resources administration and promote the balanced development of Changchun human resources.Methods:A general survey was made for health human resources allocation in 24 centers, 100 stations, totally, 124 community health service centers of Changchun five natural districts. Documentary enquiring method, interview method and questionnaire are adopted to collect information. Under the direction of the instructor and community health personnel, the questionnaire is self-designed, and through trying to survey, and then revised, and revised again, at last was used. For some contents of the questionnaire, the same questionnaire is used to survey different objects again. Investigators interview orally with investigates, and then fill answers in the form. For example, the current situation of human resources in health service stations is investigated through the executive in charge, or supervisors, to ensure that the results are reliable.The data were input and analyzed descriptively in Excel. Some of data come from the investigating results of the current situation of community health service made by Jilin Province Health Bureau in 2007 and Jilin and Changchun area annals in 2006.Contents:The method of the self-designed questionnaire and on-the-spot investigation are adopted to the allocation, the professional titleseducational background of doctors and nurses in the community, and the ratio of service population and doctors and nurses and so on.. . The analytical method of health human resources density is adopted to evaluate the balance of health human resources distribution density in each community of Changchun. The method of population ratio was used to study the current situation of health service human resources distribution. The method of tendency toward outside was adopted to calculate roughly if the quantity of health service human resources in Changchun community is different comparing with the present, if not deficient.Results :1. Unbalance of distribution of health service centers(stations) in five districts of ChangchunChangchun covers an area of 1594 square kilometers except for Shuangyang district. The total population of community health service in Changchun is 2065535, and their distribution is unbalanced. According to population density, the population density in Erdao community is large, a center or station among average twenty thousand population and 33 square kilometer, and of the total city is 10.48%. Whereas in Changyang district there is a center or station among average seventeen thousand population and 10 square kilometer, of the total city is 27.42%. For the above results it is concluded that the distribution of health service centers or stations is unbalanced in Changchun.2. The current situation and distribution of health manpower resources in Changchun communityBy the end of 2007 there are 1550 physicians and nurses in Changchun community, of them, 925 physicians and 625 nurses. The number of physicians among average thousand population is 0.45, the number of general practitioners is 0.12, the number of nurses is 0.30, of general nurses is 0.09, Therefore, the ratio between physicians and nurses is inverted.The number of physicians among average thousand population in the aspect of community in Kuancheng, Liuyuan and Chaoyang districts of Changchun is not different in general, they are respectively 0.59, 0.58 and 0.56. Whereas, in Nanguan district, it is 0.28, Erdao district, 0.10. For the number of nurses among average thousand population, in Kuancheng district the number of nurses is most, 0.50, but in Erdao, fewest, 0.09. Therefore, the number of physicians and nurses in Erdao district is obviously in the lower level.The current situation of the educational background of health manpower in Changchun community service stations or centers In the aspect of health service manpower resources in Changchun community it is lack of general physicians and nurses, especially general nurses. There are 250 general practitioners, 27.84% of the total doctors; 180 general nurses, 28.80 of the total nurses. In 1550 doctors and nurses, of them with undergraduate certificate are 277, 80%; whereas the number with secondary health educational background is dominant, 60.67%. The lower educational background of human resources in Changchun community health service is relatively lower. Therefore, it influences the quality of medical nursing and its improvement. In 1550 medical personnel, 632 people with primary professional title are 40.77% of the total medical personnel; 575 with the middle professional title, 37.10%; 198 with higher professional title are 12.77%, the constitution of professional titles is basically ladder-shaped. In 1550 medical personnel, the oldest doctor is 74 years old, the youngest is 22; average age is 49.72±13.18; the oldest nurses is 58, the youngest is 19, average age is 39.37±9.87. There are 249 male and 1301 female medical personnel, their proportion is 1:5.4.3. The number of general practitioners and nurses in Changchun community health being dominant in Jilin provinceCompared with the number of doctors and nurses among average thousand population or the general practitioners and nurses in the Jilin provincial community health manpower, the number of health manpower allocation in the Changchun community is absolutely dominant.In nine big cities in Jilin province, the number of doctors and nurses in community health in Changchun is 32% of the whole province, and among average thousand population number of doctors and nurses is also in higher level.4. The position of the allocation of health manpower resources in Changchun community Compared with Dalian, the constitution of educational background and professional titles is in the first-class level. The parallel comparison of community health service resources is carried out among Changchun, Shanghai and Dalian, it shows that the number of doctors and nurses among average population is the consistent in general. The ratio of doctors and nurses between Changchun and Shanghai is inverted. Only in Dalian the ratio is 1:1.3. In Changchun community health service centers or stations, the number of physicians with higher professional titles is 20.00, the number with middle professional titles or above is 67.46%, compared with 25.2% and 64.46 of physicians in the community health service centers in the whole country; In the eastern part of China these two norms are respectively 20.9% and 59.6%. In Changchun community health service centers or stations the number of nurses with higher professional title is 2.10%, the number with middle title or above is 23.23%; Whereas these two norms are respectively 1.9% and 22.8% in the whole country. In the east, these two norms are respectively 2.3% and 18.0%. Therefore, the constitution of professional titles of physicians and nurses in Changchun community is in the first-class level in the whole country.The survey in 2006 in Changchun shows the ratio of population general practitioners serve is 1:8000, of nurses is 1:11476; In 2003 three representative cities(Nanjing city, Wuxi city, Xuzhou city) with good community health service were chosen in Jiangsu province, in 58 health service centers and stations, the ration of the population general practitioners serve is 1:5775, of nurses is 1:6875; in Suzhou city they are respectively 1:3533 and 1:81138. In Changchun, now, community health manpower is short of general practitioners and nurse, especially, general nurses.5. The supplementing number of health service manpower resources in Changchun community up to 2010According to the method of tendency toward outside, According to the Opinions about the Developing Target of City Community Health Service in 2005 by Health Department, 2 general practitioners among every ten thousand population, it is suggested that from 2005 to 2010 the lowest allocation of nurses in community who directly offer family nursing service is not less than 3-4. By 2010 163 general practitioners and through the training of community health position, 440 or 646 general nurses will at least be supplemented.Conclusions:The human resources allocation of Changchun community health service is dominant in Jilin province, and the allocation of health resources among average thousand polppulation is in the milddle-class level in the whole country; The distribution of each community service centers(stations) in Changchun is unbalanced; the allocation of health service human resources in quantity in Changchun districts is unbalanced; the number of general practitioners in Changchun communities is also not balanced; the distribution of doctors and nurses is not balanced. The allocation of doctors and nurses among average thousand population in Changchun communities is not balanced, too. The ratio between doctors and nurses is inverted. The constitution of professional titles and educational background of human resources in Changchun community health service is relatively lower. Therefore, it influences the quality of medical nursing. Low professional titles hinder activity of their work. Compared with all other cities, the allocation of human resources in Changchun community health service is in the same level; and the constitution of educational background and professional titles of doctors and nurses in Changchun is in the first-class level in the whole country. In Changchun communities, there is a great deficiency of physicians and nurses, especially general nursing personnel.
