Aims:To investigate the recurrence pattern and its clinicopathological risk factors after radical surgery in patients with stageâ… -â…¢rectal cancer,thus to provide evidence for postoperative surveillance,preventment beforehand and treatment for local recurrent and distant metastasis.Methods:We retrospectively analyzed data of 857 patients with rectal cancer treated in a single institute during a certain period.The recurrence pattern was analyzed with SPSS statistical software package 13.0,and multivariate analysis was performed to identify the risk factors for recurrence or metastasis after radical surgery.Results:The estimated overall survival rates at 3 and 5 years in stageâ… ,â…¡, andâ…¢disease were 97.0%,89.5%;85.1%,75.5%;77.2%,60.8%, respectively;p=0.000。The common recurrent type with rectal cancer after radical surgery was local recurrence(7.8%),lung metastatis(7.0%) and liver metatasis(4.9) in order.Univariate analysis showed that factors associated with recurrence or metastasis were perioperative blood transfusion, pathological type,histopathological type,size and location of the tumor, vascular invasion,tumor nodules,T stage,N stage(p<0.05).Independent risk factors in multivariate analysis were location of the tumor,perioperative blood transfusion,vascular invasion,cancer nodes,T stage,and N stage(p< 0.05).Conclusions:These data shows that local recurrence,lung metastatis and liver metatasis is the common recurrent location after radical surgery in patients with rectal cancer.Independent risk factors for postoperative recurrence in multivariate analysis are location of the tumor,perioperative blood transfusion,vascular invasion,tumor nodules,T stage,and N stage. Multidisciplinary therapy including postoperative radiotherapy and chemotherapy is effective approach to prevent local recurrence and distant metastasis,to prolong survival.Radical surgery is the only curative therapeutic measure and thus the treatment of choice.
