Objective To investigate the changes of tumor necrosis factor-α(TNF-α),soluble tumor necrosis factor receptorsⅠ(sTNFRⅠ) and their relationship with neuron specific enolase(NSE) in cerebrospinal fluid(CSF) in children with viral encephalitis (VE),explore the pathogenesis and clinical value of sTNFRⅠin VE.Methods The level of TNF-α,sTNFRⅠand neuron specific enolase(NSE) in cerebrospinal fluid were determined by enzyme-linked immunosorbent assay(ELISA) in 55 children who were admitted with acute V E,including 25 cases with severe VE(SVE) and 30cases with mild VE(MVE).Fifteen cases with simply cryptorchidism or hernia but without VE were as control group.Comparisions between two group were used by student's test.Correlation were determinded by linear regression analysis.Results The levels of TNF-α,sTNFRⅠin cerebrospinal fluid in encephalitis children were significantly higher than that in control group,The levels of sTNFRⅠ,TNF-αin severe cases were significantly higher than in mild ones(p_α<0.01);Both the SVE and MVE patients had a lower level of sTNFRⅠ/TNFαthan that of the controls,and the SVE patients had a lower level of sTNFRⅠ/TNFαthan that of the MVE ones (p_α<0.01);sTNFRⅠ/TNF-αwas negatively correlated with NSE(P<0.01),sTNFRⅠand TNF-αwere positively correlated with NSE(p_α<0.05);The correlated coefficient of TNF-αand sTNFRⅠin SVE patients was lower than that of the MVE ones.Conclusion The level of TNF-α,sTNFRⅠand their ratio in cerebrospinal fluid of children with virus encephalitis have closely relationship with pathogenetic condition; The deficiency of sTNFRⅠcompared with the high level of TNFαplay an important role in the pathogenesis of viral encephalitis patients,especially severe ones.
