PartⅠAn Investigation of Efficacy and Safety of Arginine and Zinc Sulfate Capsule in the Treatment of Asthenospermia andOligospermiaExperiment one An Investigation of Efficacy of Arginine and Zinc Sulfate Capsule in the Treatment of Patients with asthenospermia and oligospermiaObjective To assess the effect of arginine and zinc sulfate capsule in the treatment of asthenospermia and oligospermia.Methods 170 asthenozoospermic and oligoasthenozoospermic patients, who were willing to take arginine and zinc sulfate capsule for treatment, were randomly divided into two groups in a ratio of 3:2: arginine and zinc sulfate capsule test group(n=100), placebo controll group(n=70). All the drugs were orally taken. Three capsules were taken three times a day. They will continually take 2700 mg arginine and zinc sulfate capsule per day for 80 days. Subjects were asked to record their diary and adverse drug reactions as capsules intake. Three clinic visits were scheduled during the treatment phase on the 30th, 60th, and 90th day, and the normal sperm quality was analyzed at the same time. Gametes of subjects were checked HCG in urine before treatment and after 90 days. The effects of the drug were estimated after 90-day treatment.Results1. There was no statistically significance in two groups including age,height,weight,weight index,smoke,nation,the age of marriage,the time of infertility,sexy,body examination (P>0.05).2. There was no statistically significance in two groups including semen fluid volume,sperm morphology,sperm density,sperm vitality,sperm viability,a grade sperm,a and b grade sperm before treatment (P>0.05).3. The effect ratios in the two groups of 30-day, 60-day and 90-day treatment were 45.13%, 60.47% and 62.54%, respectively. The result demonstrated the effect ratio of the test group was obviously higher compared with that in the placebo group, and there was statistically significance (P<0.01). The natural pregnancy condition during treatment: 3 cases were in the test group, 1 case was in the placebo group.Conclusion The effect ratio of arginine and zinc sulfate capsule to treat patients with asthenozoospermia and oligoasthenozoospermia could reach to 62.54% after the drugs were continually taken 80 days ,900 mg capsules each time, three times a day. Experiment two A Estimation of Safety of Arginine and Zinc Sulfate Capsule in the Treatment of Patients with asthenospermia and oligospermiaObjective To assess the safety of arginine and zinc sulfate capsule in the treatment of asthenospermia and oligospermia.Methods The semen samples in partⅠwere collected before treatment and after 30-day,60-day and 90-day treatment, and the blood and urine samples were collected before treatment and after 90-day treatment. All subjects were examined about laboratory examination and the zinc concentrations in their seminal plasma, spermatozoa and serum were detected by flame atomic absorption spectroscopy. Results1.There was no statistically significance of the zinc concentrations in the seminal plasma, spermatozoa and serum in two groups before treatment (P>0.05);There was statistically significance of the spermatozoa zinc contents in the test group before and after treatment (90th-day to 0-day)(P<0.05),but not in the placebo group (P>0.05); There was no statistically significance of the seminal zinc contents in the two groups before and after treatment (P>0.05); There was statistically significance of the serum zinc contents both in two groups before and after treatment(90th-day to 0-day)(P<0.01).2.The results of the laboratory examination were analyzed both groups, low difference ratio were before and after treatment. The ALT of 22 subjects were slightly abnormal after treatment: 16 cases in the test group (16 percent), 6 cases in the placebo group (8.72 percent), the highest level of ALT of test group and placebo group were 121U/L and 172 U/L ,respectively. 7 cases of them were no more than 60 U/L.3. No serious adverse reaction happened. Conclusion It was little that the adverse drug reaction of arginine and zinc sulfate capsule. PartⅡPreliminary Explore the Mechanisms of Arginine and Zinc Sulfate CapsuleExperiment one Effect of Arginine and Zinc Sulfate Capsule on Reproductive HormoneObjective To observe the effect of arginine and zinc sulfate capsule on reproductive hormone.Methods The serum samples of the test group in partⅠwere collected before and after treatment, the concentrations of follicle stimulating hormone(FSH), luteinizing hormone(LH) and testosterone(T) in serum were detected by radioimmunoassay.Results There was no statistically significance of all the reproductive hormone (FSH, LH, T) in the test group before and after treatment(P>0.05).Conclusion Asthenozoospermic and oligoasthenozoospermic patients'sperm quality was improved by arginine and zinc sulfate capsule via non-hormone pathyway or indirect hormone pathyway. Experiment two Effects of Arginine on Sperm Motility Parameters in Patients with AsthenospermiaObjective To observe the effect of arginine on sperm motility parameters and acrosome reaction in patients with asthenozoospermia in vitro, analyze the relationship between sperm motility parameters and the production of nitric oxide Level.Methods 20 semen samples of asthenozoospermia donors were co-incubated with different concentrations of arginine(2.5mM,10mM,40mM)and 1μM progesterone (four medicated group and a negative control group). Sperm motility parameters such as velocity straight linear (VSL), velocity curve linear (VCL), velocity average path (VAP), amplitude of lateral head displacement (ALH) and progressive motility were analyzed with computer-aided semen analysis (CASA) at 10min, 30min, 60min and 120min after co-incubation, respectively. Sperm viability was checked by eosin Y trial. Nitric oxide concentrations in the seminal plasma and spermatozoa at different time were measured by ultraviolet spectrophotometry, and sperm acrosome reaction ratios in the negative control group, 2.5mM arginine group, 10mM arginine group and progesterone group were detected at 0min and 60min by the Commassie Blue Fast (G250) Staining method.Results1.Compared with those in the control group, Sperm motility parameters in 2.5mM and 10mM arginine group were obviously improved at 30, 60 and 120min after co-incubation (P<0.05), but inhibited in the 40mM arginine group in the same time (P<0.05 or P<0.01 ).2. There was no obvious difference of sperm viability in all arginine groups and control group.3. Seminal plasma nitric oxide concentration in the 2.5mM and 10mM arginine group was higher than that in the control group at 60 and 30min, but there was no statistically significance; the nitric oxide concentration in seminal plasma of the 40mM arginine group were obviously higher than that of the control group at 60 and 120min after co-incubation (P<0.05).4. There was a significantly increased in the ratio of sperm acrosome reaction in the 2.5mM, 10mM arginine group and progesterone group with comparison that of the control group at 60 min after co-incubation (P<0.05), but there was no statistically significance between 2.5 mM, 10mM arginine and progesterone.Conclusion Sperm motility and acrosome reaction of asthenospermic patients could be improved in vitro by arginine, the stimulation of the drug occurred in dose-dependent manner. Sperm motility can be improved when the concentrations of nitric oxide were 1μΜto 40μΜ, but inhibited when the concentrations of nitric oxide were more than 40μΜ.
