Background:In recent years,with the changes of people's living habits and diet,the incidence of colorectal cancer have a clear upward trend.Carry out molecular pathological study and reveal the biological proceeding of colorectal carcinoma is significant to colorectal carcinoma's diagnose and healing.Now it becomes the hot spot of the tumor study.Detecting tumor markers by Immunohistochemistry(IHC) is a convenient and economical way which is also mature in technology and easy to lunch, and be in common use.This reseach's Purpose is to detect the expression of CK20(Cytokeratin20) and EMA(Epithelial membrane antigen) in colorectal carcinoma by IHC and discuss it's clinical significance.Objective:1,To investigate the expression and the characteristic of EMA and CK20 in the colorectal cancer tissue,and analysis the relevance with colorectal cancer DUKES staging,the degree of differntiation,and the metastsis of lymph node.2,To explore the intensity of expression and the characteristic of associated expression of EMA and CK20,in order to guide the clinical pathology diagnosis and the prognosis of colorectal cancer.Method:To detect the expession of EMA and CK20 in 50 colorectal cancer tissue samples by immunohistochemistry technique used Maxvision kit.And analysis the relevance between the expression and colorectal cancer DUKES staging,the degree of differentiation,soakage depth and the metastsis of lymph node,so that we could guide the clinical pathology diagnosis and the prognosis of colorectal cancer.The contrast of EMA and CK20 interblock positive rate used chi-square statistics.The correlation analysis of the expression of EMA and CK20 used spearson test.The analysis of the expression of EMA and CK20 and the clinical pathology characteristic used Kruskal-Wallis H test.And the relationship of the associated expression of EMA and CK20 and the clinical pathology characteristic used chi-square statistics.Result:EMA and CK20 both express in the cytomembrane and the cytoplasm,and both of them have heterogeneity.The total positive rate of EMA is 76.0%,and CK20 is 36.0%.The positive rate of EMA is not associated with the distinguishing between the sexes,ages and clinical stage,but it is associated with the degree of differentiation (P<0.05).The positive rate of CK20 is associated with the tumor stages,the degree of differentiation,soakage depth and the metastsis of lymph node(P<0.05).The intensity of expression of EMA is not associated with the tumor stages,soakage depth and the metastsis of lymph node,but it is associated with the degree of differentiation,and it increases along with the reduce of the degree of differentiation;the intensity of expression of CK20 is not associated with the soakage depth,but it is associated with clinical staging,the degree of differentiation and metastsis of lymph node,it increases along with the increase of clinical staging,the dawn of differentiation,and the metastsis of lymph node.The intensity of expression of EMA have inverse correlation with the expression of CK20.In the associated detection,the CK20(-)/EMA(+) patterns of expression has highest expression rate(60%),and the CK20(-)/EMA(-) patterns of expression has lowest expression rate(4%).Conclusion:1,The expression in colorectal cancer of EMA is associated with the degree of differentiation,higher the degree of differentiation of colorectal cancer,and higher the positive rate of expression of EMA,and lower conversely.2,The positive rate of expression of CK20 is associated with clinical staging,the degree of differentiation,the deepen of soakage,and the metastsis of lymph node.It increases along with the increase of clinical staging,the induce of the degree of differentiation,the deepen of soakage and the metastsis of lymph node.But the total rate is lower,we can't regard it as a helpful biological indicator.3,The intensity of expression of EMA is not associated with the tumor stages, soakage depth and the metastsis of lymph node,but it is associated with the degree of differentiation,and it increases along with the reduce of the degree of differentiation; the intensity of expression of CK20 is not associated with the soakage depth,but it is associated with clinical staging,the degree of differentiation and metastsis of lymph node,it increases along with the increase of clinical staging,the dawn of differentiation,and the metastsis of lymph node.The intensity of expression of EMA has inverse correlation with the expression of CK20,and both the expression of EMA and CK20 increased along with the increase of clinical stage.In the associated detection,the CK20(-)/EMA(+) patterns of expression maybe give directions to the sensitivity and specificity of the colorectal cancer diagnosis.The CK20(+)/EMA(-) patterns of expression is associated with DUKES stage,and the CK20(+)/EMA(+) patterns of expression is associated with the metastsis of lymph node,so that the CK20(+)/EMA(+) patterns of expression maybe indicate the metastsis of lymph node and a poor prognosis.The CK20(+)/EMA(-) patterns of expression is associated with the metastsis of lymph node,so that it maybe indicate that the patient have no metastsis of lymph node,and his DUKES stage maybe at A or B,and a better prognosis.
