Purpose:To study the clinic value of contrast-enhanced transrectal ultrasound in the categoryⅢprostatitis.So that,evaluate it' s function on diagnosis and judgment of curative effect,as well as provide evidence of the imaging for the clinical diagnosis of categoryⅢprostatitis.Materials and Methods:CategoryⅢprostatitis:45 male patients.Of these,21 cases were categoryⅢA prostatitis,21cases were categoryⅢB prostatitis.They had been no theraphy before the ultrasound examination.15 cases could be control group.All patients' prostatic fluid were tested under microscope and bacterial culture.They had checked by transabdominal ultrasonography and filled in the NIH-CPSI questionaire before examination by transrectal ultrasound.The investment was Philips-IU22 color-Doppler ultrasound diagnostic unit which had PIHI and QLab(Region of Interest Quantification Plug-in/Tool).The constrast medium was SF6 micro vesicle soliquoid which was combined by 4ml saline and Sono Vue-Ⅱ(Bracco co.).Carry on color Doppler transrectal ultrasound at first,to observe the volume,peplos,interior echo,blood shape,blood number and blood distribution of the prostate and look for the richest section of the blood stream,calculate the blood vessel number.Take the most remarkable bloodstream of the inner and outer gland with PW to judge the blood signal nature,observe the blood fow frequency spectrum shope,carry on spectral analysis,get out the blood stream dynamics parameterss (PSVmax,EDVmin and RI),take average values of three times.Then inject the constrast agent and observe the feature of infusing as well as saved the dynamic and quiet images.Retrospective analyse the ultrasound visualization images feature and the AT,TTP,BI and PI by QLab of 10 categoryⅢprostatitis which confirmed by clinical and laboratory examination.The aim is to find the categoryⅢprostatitis ultrasound visualization characters by compared with normal prostate ultrasound visualization images.Results:(1) The volume and vascularity grading of prostate had statistically difference between the patients groups and the control group(P≤0.05).(2) Calcification in the prostate had no statistically difference between the patients groups and the control group(P>0.05).(3) The NIH-CPSI score,the pain index,the symptom of urinary tract and the quality of life had no statistically difference betweenⅢA andⅢB group(P>0.05).(4) The PSVmax and EDVmin of the areas within the inner gland had statistically difference between the patients groups and the control group(P≤0.05);The EDVmin of the outer gland and the RI of the areas within the inner gland had no statistically difference between the patients groups and the control group(P>0.05);The PSVmax and the RI of the outer prostate had statistically difference between the patients groups and the control group(P≤0.05).(5) The AT,TTP and△A had no statistically difference between the patients groups and the control group(P>0.05);The AT and TTP of the outer gland shorter than the the area within the inner gland.The△A of the area of within the gland higher than the area of outside.Conclusions:Transrectal ultrasound could clearly display the volume,the peplos,inner structure,internal echo,calcification and blood feature of the prostate.The essay showed there were statistically difference in volume,blood class,the PSVmax and EDVmin of the areas within the gland between the patients groups and the control group.This had worth to be research deeply.The essay showed there were some overlapping in the perfusion character between the categoryⅢprostatitis and the normal prostate.We had no full reliable evidence to clinical diagnosis and evaluate the curative effect of categoryⅢprostatitis.This had to be approached further until we had adequate cases.
