Study On The Syndromes Evolutive Law Of TCM In Advanced Colorectal Cancer During Chemotheraphy

Objective:To explore the clinical differentiation of syndromes character and evolvement law during the chemotheraphy of advanced colorectal cancer.To provide theoretical evidence of Chinese traditional medicine enhancing ability of disease treatment during the chemotheraphy of advanced colorectal cancer.Method:The clinic TCM data of advanced colorectal cancer patients during the chemotheraphy were collected,and database was constructed.Summarized the syndromes character with descriptive statistical analysis and principal symptoms and syndromes diagnosis of TCM according to the clinic and the basic theoretics of TCM.Found out the clinical differentiation of syndromes character using McNemar tests.Then summarized the syndrome complex and evolvement Law during the chemotheraphy of advanced colorectal cancer.Results:According to the descriptive statistical analysis,the principal symptoms of advanced colorectal cancer were:lassitude of spirit and lack of strength, torpid intake,lackluster of complexion,thirst,weakliness of waist and knees, hair losing and tooth rocking,inability to sleep,vexation,anxiety,sloppy stool,diarrhea,hematochezia,feeling windy,feeling anus prolapse,slightly pain at abdomen,tumour in abdomen,feeling pain at waist and sacrum at all times,lackluster tongue,red tongue,purple tongue,petechia tongue,enlarged tongue,teeth-marked tongue,thin fur,slightly thick fur,thick fur,white fur,moist fur,slimy fur,slightly slimy fur,waterline tongue,fine pulse, string-like pulse,slippery pulse,deep pulse,ect.According to the McNemar tests,during or after the chemotheraphy,the evolutive symptoms statistically significant were:lassitude of spirit and lack of strength,faint low voice, numbness of the limbs,spontaneous sweating,dry mouth but only wanting to gargle,torpid intake,nausea,vomit,dry stool;purple tongue,petechia tongue, teeth-marked tongue,slightly thick fur,yellow fur,yellow-white fur,slightly slimy fur,deep pulse,which wereincreasing(P＜0.05).And thin fur,white fur, moist fur,were decreasing(P＜0.05).According to the results of the descriptive statistical analysis and the present recognized classify standard of syndromes diagnosis of TCM,advanced colorental cancer could be sum up to six syndrome types:the Spleen deficiency with dampnes and blood stasis pattern,the Spleen deficiency with dampnes to xin pattern,the Spleen-Kidney deficiency pattern, the Liver-Kidney Yin deficiency pattern,the dual deficiency of Qi and blood pattern,the Spleen-Stomach disharmony pattern.Before chemotheraphy,the Spleen deficiency with dampnes and blood stasis pattern accounted for 71.7%. After chemotheraphy,the Spleen deficiency with dampnes and blood stasis pattern accounted for 41.7%,while the Spleen-Stomach disharmony pattern accounted for 36.7%.Conclusions:The syndromes character of advanced colorental cancer is that Deficiency and Overabundance are existing at the same patient and at the same time.Qi, Blood,Fluid and Humor metabolize abnormally have an important effect on the disease.The key to mechanism of disease is dampness,blood stasis and deficiency.The disfunction of the Spleen and the Kidney play the great role, and the Liver and the Lung is involved.Early after the chemotheraphy,the Spleen deficiency with dampnes and blood stasis pattern increases distinctly, and the Qi deficiency syndrome,phlegm-dampness syndrome,blood stasis syndrome aggravate.To enhance ability of disease treatment,Chinese therapeutic methods of harmonizing Spleen and the Stomach,replenishing Kidney,tonifying Qi,dispelling phlegm,activating blood can be used.
