Backgroud Lung ischemia -reperfusion injury(LIRI)is still a challenge for the thoracic surgeon,which occurs in many clinical conditions,such as cardiopulmonary bypass,lung embolism,shock etc,especially during the lung transplantation.Those may lead to acute lung injury or even multiple organ dysfunction syndrome(MODS). ischemia -reperfusion induced lung injury contributes to a significant cause of morbidity and mortality after the lung transplantation,so it is necessary to better understand the mechanisms of Lung ischemia -reperfusion injury and investigate more reliable strategies to reduce the ineidence of the injury.Objective To investigate the effect of adriamycin on heme oxygenase-1 in a rats lung ischemia -reperfusion injury.Methods 32 healthy male Wistar rats were dividedrandomly into four groups (eight rat s in each group) surgical controls (Cgroup),Adriamycin group(ADM group), ischemia-reperfusion group (IR group) and ADM+IR group(ADM-IR group). The unilateral lung ischemia-reperfusion model was replicated, Rats from each group were sacrificed in 45 minutes of lung–clamping followed by 2 hours of reperfusion , harvested for theright lung and detected for histology.The dry /wet weightratio of lung tissue (D/W), injured alveolus rate (IAR), the activity of HO-1, the expression of HO-1 protein were measured respectively in different groups. The data were processed with SPSS13.0Results 1. The D/W ratio of lung tissue: The D/W ratio of lung tissue in the IR group and ADM-IR group were lower than that in the C group(P<0.01),but significantly higher than that in the IR group(P<0.01). but there was no significant difference between ADM group and normal group( P >0.05).2. IAR: IAR were much higher in IR group and ADM-IR group than that in C group and ADM group(P<0.01),but in ADM-IR group, the indexes were significant lower than those in IR group(P<0.01),but there was no significant difference between ADM group and normal group( P >0.05).3. Histological evaluation: In the process of ischemia-reperfusion ,the lung injury was aggravating progressively in the IR group; there was marked pulmonary capillary congestion, interstital edema and intraalveolar hemorrhage, infiltration; The pulmonary pathologic alterations occurred to a lesser degree in the ADM-IR group compared with the IR group.4. The activity of HO-1and the expression of HO-1 protein: The activity of HO-1,the expression of HO-1 protein were much higher in ADM group ,IR group and ADM -IR group than those in C group(P<0.01),and the indexes in ADM-IR group increased more significantly(P <0.01).Conclusions 1."The lung ischemia-reperfusion injury model"was established successfully in the rats. 2.Adriamycin can improve the activity of HO-1,up-regulate the expression of HO-1 protein,so that it can ameliorate LIRI in rats significantly by activating the HO-1 in lung tissue .3.low dosage of Adriamycin does little harm to rat lung .
