Objective:Observing the effect of Buyanghuanwu on the index of FBG,NO,Upro,ICAM-1 and VCAM-1 of rats with the experimentale diabetes,along with the purpose of discussing the recipe of Qi-invigorating and Blood-activation Therapy's protective role on kidney injured by diabetes,then it is to provide scientific and experimental evidences for the clinical application of the recipe.Methods:100 healthy male SD rats were adopted and fed adaptively,and were separated as compared group and model group randomly.The compared group was fed with standard fodder,the model group was fed with high sugar and high fat fodder.Both for 4 weeks,then the both tow groups were fasted for 12 hours and aperitoneally injected with small dose streptozotocin copy diabetic models.According to the theory in TCM such as overstrain causing qi exhausetion and qi asthenia causing blood stasis,the rats in model group were swimming in water of(37±0.5)℃with the depth of 45cm.The rats were fished out of water when they nearly drowned feeding with high sugar and high fat fodder.Doing this for 2 weeks,the rats in model group became deficiency of qi.At the same time,rats in compared group swam appropriately.Then the modeled rats were divided into M group,D group,B group,I group and C group,5 groups in total.The rats were orally administered for 8 weeks and in the eighth week,we remained 24 hour urine in order to detect 24h proteinuria excretion.Tube Feeding with medication,after the last time feeding,they were fasted for 12 hours and then weighed.After the rats were killed,We separated blood serum and plasma to test their fasting blood glucose,NO. The rats were taken left kidneys by strict aseptic manipulation, the rats' kidney was weight and calculated the KW/BW index after the renal capsule was dissected.The renal tissue was fixed, it was analyzed by light microscopy and studied by immunohistochemistry.Results:1 The effect of Bu Yang Huan Wu Tang on FPGCompared with the C group,FBG of rats in M group was increased significantly(p<0.01);compared with M group,FBG of rats in treatment groups were decreased evidently(p<0.01); compared with D group and B group,FBG in I group has significant difference(P<0.05,P<0.01).2 The effect of Bu Yang Huan Wu Tang on body weight, KW/BWWhen these drugs are effective in the beginning,weight of rats in the rest groups have no significant difference except C group(P>0.05);At week 4 and week 8,compared with M group, weight of rats in treatment group was decreased evidently (P<0.05,P<0.01);compared with B group and D group,weight of rats in I group was decreased evidently(P<0.05,P<0.01).Kidney hypertrophy(KW/BW) Index of rats in M group was significantly higher than those in C group(P<0.01); Compared with M group,kidney hypertrophy index of rats in treatment group were decreased evidently(P<0.01);Compared with B group and D group,kidney hypertrophy index of rats in I group has significant difference(P<0.05,P<0.01).3 The effect of Bu Yang Huan Wu Tang on 24h urinary proteinThe 24h urinary protein quantitative of the rats in M group was significantly higher than that of C group(P<0.01); compared with M group,the 24h urinary protein quantitative of treatment group was obviously lower(P<0.01);There was difference among I group,B group and D group(P<0.05, P<0.01)4 The effect of Bu Yang Huan Wu Tang on NOCompared with the C group,the NO of rats in M group was increased significantly(P<0.01);compared with M group,NO of rats in treatment groups were decreased evidently(P<0.01); There was difference between I group and D group(P<0.05), There was no difference between I group and B group(P>0.05).5 The pathomorphology change of kidney by Light microscope observationAfter stained with HE under light microscope C group: renal tubule,glomerular capillary loops and mesangial cell are normal.M group:degeneration,dropsy,falling in renal tubular epithelial cells,glomerular hypertrophy,mesangial cell proliferation;Compared with M group,renal tubular epithelial cell injury significantly reduced.6 Expression of ICAM-1,VCAM-1 in renal tissue examined by immunohistochemistryICAM-1 in cells of glomerulus and nephridial tubule were expressed slightly in group C(0.0334±0.0248),but in group M (0.2761±0.0419),the differences were evident.Expression of kidney ICAM-1 in three treating groups(D:0.2085±0.0232, B:0.1678±0.0346,I:0.1252±0.0283) had great differences compared with group M(P<0.01).ICAM-1 in group I had decreased compared with group D and B(P<0.01).The expression of VCAM-1 in M group was markedly enhanced(0.4270±0.0647),compared with the C group (0.0351±0.0249),has significant difference(P<0.01);compared with the model group,the expression of VC AM-1 in D group (0.2696±0.0444) and B group(0.1992±0.0443) and I group (0.1502±0.0486) reduced significantly(P<0.01),compared with the D group and B group,the expression of VCAM-1 in I group has significant difference(P<0.01).Conclusion:1 The rats were given a low dose streptozotocin intravenously and fed with high calorie laboratory caw to establish the murine model of diabetes mellitus.the expression of NO,ICAM-1,VCAM-1 in diabetic rats is abnormal.2 Buyang Huanwu Decoction can lower the index number of kidney hypertrophy(KW/BW) and the alleviate injury of the rats'kidney induced by proteinuria.3 Buyang Huanwu Decoction can lower the quantity of nitric oxide and reduce the damage of the kidneys.4 Buyang Huanwu Decoction can down-regulate the exprssion of ICAM-1,VCAM-1 in renal tissue,which interfere with the development of diabetic kidney disease.5 The effect of combination therapy group is superior to simple use of Chinese medicine treatment group and western medicine treatment group.
