Effect Of Different Oral Hypoglycemic Agents On Glycaemic Control And Pancreatic β Cell Function In Type 2 Diabetes

Objective:To observe the effect of metformin combined with insulin secretion promoting agents or insulin sensitizer on the glycemic control and pancreaticβcell function in type 2 diabetes patients.Methods:88 patients with type 2 diabetes were enrolled from the Diabetes Information Management System(DIMS) of The second Affiliated Hospital of Zhejiang University School of Medicine,who had been follewed up for more than one year and evaluated theirβcell function twice.All of them were treated with oral antidiabetic drugs,of which 47 were treated with insulin secretion promoting agents plus metformin(Group A),and 41 with insulin sensitizer plus metformin(Group B).The Fasting blood glucose (FBG),2 hours postprandial blood glucose(2hPBG),Glycosylated hemoglobin Alc (HbAlc),Fasting insulin(FINS),2 hours postprandial insulin(2bINS),HOMA-IR,HOMA-IS,HOMA-IAI were tested,analyzed and compared before and after the enrollment of each group,also compared between the two groups.Results:After one-year treatment,the FPG,2hPBG,HbAlc of both groups were decreased significantly(Group A:141.5±38.51 vs 121.1±16.07,235.1±88.37 vs 161.8±44.89,7.1±1.57 vs 6.3±0.55;Group B:132.1±36.53 vs 122.2±21.47,216.49±79.89 vs 172.7±56.69,6.8±1.57 vs 6.2±0.52,P<0.05),the successful control of HbAlc increased from 48.94%to 65.96%and 52.38%to 71.43%,separately. HOMA-IS increased significantly(50.98±42.595 vs 53.42±28.976,P<0.05) in Group A;HOMA-IR decreased and HOMA-IAI increased significantly(4.10±2.654 vs 3.16±2.095,0.01,0.009 vs 0.02±0.012,P<0.05) in Group B.There were no statistically significance in all these parameters between the two groups after one year.Conclusion:Treatment with metformin combined with insulin secretion promoting agents or insulin sensitizer were both significantly effective in lowering glucose levels and improving the successful control of HbAlc.The insulin secretion promoting agents increased the insulin secretion and insulin sensitizer increased the insulin sensitivity.
