ObjectiveEvaluate the cognitive pattern of patients suffered from panic disorder in general hospital.After that,give them cognitive-behavioral therapy aimed at the cognitive pattern,and then evaluate the contribution of it to the cognitive pattern.SubjectsTake 89 patients suffered form panic disorder who came to out-patient department of psychology in Shengjing hospital of China Medical University as therapeutic group, 50 normal people as normal controls,and 28 patients suffered from panic disorder as waiting group.Methods1.Evaluation:Using the automatic thoughts questionnaire(ATQ),illness behavior questionnaire(IBQ),dysfunctional attitudes scale(DAS) and Hamilton anxiety scale(HAMA),Hamilton depression rating scale for depression(HRSD,17 items)to evaluate 117 patients with panic disorder and 50 normal controls before taking any therapy.Using ATQ,IBQ,DAS and HAMA to evaluate the patients with panic disorder after taking therapies.Gather the statistics and analyze the baseline of the patients and the normal controls,and the decrease deduction of the patients in each therapeutic and waiting group.2.Structured Clinical Interview for DSM-Ⅳ,SCID3.Therapies supplying:Specially supply cognitive-behavioral therapy,paroxetine therapy,and combination therapy to the patients in each group respectively.4.statistics:To evaluate the baseline of patients suffered from panic disorder and the normal controls;to evaluate the change of the decrease deduction of each therapeutic group and the inter-group difference of decrease deduction. Results1.The feature of cognitive pattern in the patients with panic disorderCompared with normal controls,the scores of ATQ are significantly higher(P<0. 01);In IBQ evaluation,the scores of almost all of the factors are significantly higher than normal controls(P<0.01) except the P/S factor(P>0.05);In DAS evaluation, the total scores and almost all of the factors scores obtained from the patients are higher than normal controls(P<0.01 or P<0.05)except the scores of cognitive philosophy.2.The contribution of each therapy to the cognitive patternAfter 8 weeks' long treatment,the scores of ATQ obtained from the patients in each therapeutic group significantly decreased(P<0.01),the scores of ATQ obtained from the patients in cognitive-behavioral therapeutic and combination therapeutic groups significantly decreased than the patients in paroxetine therapeutic group(P<0.05).In IBQ evaluation,the scores of GH,DC,AD,I,WI,AS,DA factors obtained from the patients in cognitive-behavioral therapeutic and combination therapeutic group significantly decreased(P<0.01),while there are no difference in the other factors(P>0.05);the scores of GH,AD,I,AS factors obtained from the patients in paroxetine therapeutic group significantly decreased(P<0.01),while there are no difference in the other factors(P>0.05).In DAS evaluation,the total scores and each factor scores obtained from the patients in each therapeutic groups didn't decrease significantly(P>0.05).3.The situation and variation of the patients' emotional evaluationThe scores of HAMA and HRSD obtained from the patients suffered from panic disorder are significantly higher,(HAMA,28.37±5.71;81 patients' HRSD scores are more than 17).After 8 weeks' long treatment,the scores of HAMA obtained from the patients in each therapeutic group significantly decreased(P<0.01) with no significant difference(P>0.05).ConclusionsThere are similar cognitive pattern in the patients with panic disorder and depression.Compare to paroxetine therapy,cognitive-behavioral therapy and combination therapy can significantly improve the cognitive pattern including correct the negative cognition and the feeling of illness.All of the 3 therapies including cognitive-behavioral therapy,paroxetine therapy and combination therapy on the patients suffered from panic disorder are effective to control the anxiety with no significant difference.
