ObjectiveDuplicate the model of the rat small intestine ischemia-reperfusion,observing the influence of Hyperbaric oxygen(HBO) on the ischemia-reperfusion injury of rat small intestine,to compare the protective effect of HBO on rat intestinal mucosal barrier on each phase target,and then to clarify the protection mechanism by selecting the optimal of the application of HBO.MethodsThe superior mesenteric artery(SMA) was occluded and then released to produce the intestinal ischemia-reperfusion model in rats.Forty male SD rats were divided into four groups randomly:①Ischemia-reperfusion group(I-R):Anesthesia planing exposed abdominal superior mesenteric artery,clamping cause ischemic one hour, then unclamping to reperfuse one hour;②Hyperbaric oxygen preconditioning(HBO-P): After pre-giving one hour hyperbaric oxygen,then I-R one hour.③Ischemia group hyperbaric oxygen(HBO-I):Anesthesia planed one-hour period of abdominal ischemia simultaneously give hyperbaric oxygen,then reperfuse one hour;④Reperfusion period of hyperbaric oxygen group(HBO-R);Anesthesia planing abdominal ischemia one hour,one-hour period of reperfusion to hyperbaric oxygen.Taking intestinal tissues of each group in accordance with its corresponding points.Detection of intestinal tissue homogenate TNF-αby using enzyme-linked immunosorbent assay method(Elisa). Measured I125 mark insulin(molecular weight 5000Da) from intestines mucous to serosa Counts Per Minute by using flip pocket Act,which is to obtain the permeability of intestinal mucosa.Morphological changes were observed in rat intestinal mucosal under the light microscope when it was Conventional HE dye.The damage of small intestine organization carries on grading using the Parks grading method.And then to observe ultrastructural changes of intestinal mucosal epithelial cells under transmission electron microscope.ResultsThe mucosal hyper permeability of the gut sac after I-R was significantly ameliorated in HBO-I group(P<0.05) in comparison with HBO-P groups,in HBO-P group(P<0.05) in comparison with HBO-R and I-R groups,with no significantly difference between HBO-R and I-R group(P>0.05);TNF-αlevel in HBO-I group was significantly lower comparision with that in HBO-P group(P<0.05),in HBO-P group (P<0.05) comparision with that in HBO-R and I-R groups(P<0.05),with no significantly difference between HBO-R and I-R group(P>0.05);The histological injury scores of intestinal tissue in HBO-I was significantly lower than that in HBO-P (P<0.05),and also significantly lower in HBO-P(P<0.05) comparision with that in HBO-R and I-R groups(P<0.05),with no significantly difference between HBO-R and I-R group(P>0.05).the ultrastructural changes of rat intestinal epithelial cells under Electron microscopy:the worst intestinal microvilli was lost,mitochondrial vacuole, degeneration,cristae disappearance,the top of tight junctions was separated in I-R group.And in HBO-I group,the best intestinal microvilli was normal,mitochondria normal more or less,the top of tight junctions was closed;In HBO-P group,some intestinal microvilli was lost,mitochondrial vacuolization and degeneration,cristae disappearance,the top of tight junctions was opened slightly;in HBO-R group, intestinal microvilli was lost substantially,mitochondrial vacuolization and degeneration,cristae disappearance,the top of tight junctions was separated. ConclusionHBO can increase oxygen tension at injured intestinal mucosa organizations,and increase reserves of the oxygen,in order to change lack of organization oxygen;being able to speed up the blood flow velocity and inhibit hemagglutination system to protect the intestinal mucosa from ischemia-reperfusion injury effectively.This research further found that HBO could reduce the production of inflammatory gene TNF-α,in post-ischemic intestinal tissue,and reduce the excessive inflammatory response;It can also improve the intestinal tissue pathological damage,and reduce the permeability of intestinal mucosa barrier;In short,the application of HBO therapy reduced significantly small intestinal permeability in HBO-I group and HBO-P group,inhibited production of TNF-α,and protected effectively the small intestinal mucosal barrier.The application of HBO therapy have effect on HBO-R(comparised I-R),but no significant statistically.
