Objective: To study on distributing characteristic of symptoms in Chinese medicine of chronic Hepatitis B, And study on the relationship among the symptom,virology of chronic Hepatitis B and T lymphocyte subsets.Methods: One hundred and twenty subjects with CHB were divided into five types according to the syndrome dieffrentiation of Trodational Chinese Medince: 1. liver stagnation and spleen deficiency 2. dampness-heat accumulation 3. blood-stasis in meridian 4. liver-kidney yin deficiency 5. spleen-kidney yang deficieney. Then research their gender,age,the time they got this disease. At last, examining their HbsAg ,HbeAg, HBV-DNA, CD4~+ and CD8~+ to find the difference and the relationship among them.Results:1. The distributing-rule of TCM symptom of Chronic Hepatitis B is: liver stagnation and spleen deficiency > dampness- heat accumulation >blood-stasis in meridian >liver-kidney yin deficiency > spleen-kidney yang deficiency. There is no obvious difference about the rate of gender among five groups; liver stagnation and spleen deficiency patients had the smaller age to other groups (P<0.05); liver stagnation and spleen deficiency patients had the shortest disease history among all the groups(P<0.05).2. In all the groups, dampness-heat accumulation has the highest HBeAg-positive rate, comparing with other groups , the difference is obvious (P<0.05). There is no obvious difference among liver stagnation and spleen deficiency,blood-stasis in meridian and liver-kidney yin deficiency (P>0.05). HBsAg-positive rate in all symptoms patients does not change ob-viously(P>0.05)3. In all groups, the positive rate of HBV-DNA in dampness-heat accumulation group is highest, blood-stasis in meridian to be the lowest, comparing these two groups, there is the the significance difference between them (P<0.05). Comparing with other groups, there is no significant difference (P>0.05).4. CD4~+ and CD4~+ / CD8~+ of the patients of liver stagnation and spleen deficiency,blood-stasis in meridian and liver-kidney yin deficiency decreases in varying degrees. Comparing these three groups with dampness -heat accumulation group and the control group, there is significant difference(P<0.05). CD8~+ in all groups has little difference (P>0.05).5. Comparing HBeAg (+) group with HBeAg (-) group, CD4~+, CD8~+, CD4~+ / CD8~+ were not significantly different(P>0.05).Conclusion:In clinical work , patients of chronic hepatitis B with liver stagnation and spleen deficiency were to be the most common type. Also in patients with spleen deficiency and liver stagnation had the shortest course of disease and the patients were the youngest. Therefore stagnation of liver-type spleen deficiency with chronic hepatitis B may be the early stage of the disease progression. Type of dampness-heat accumulation in patients with high viral replication, Hepatitis B is a typical activity. liver stagnation and spleen deficiency, liver kidney-yin deficiency type, blood-stasis in meridian in patients with CD4~+, CD4~+ / CD8~+ were reduced to varying degrees, but signifycantly lower than those in the control group, patients with dampness-heat accumulation of CD4~+ CD8~+ do not change obviously.
