Objective:An intestinal ischemia reperfusion(I/R) rat model was adopted by blocking the SMA to observe the morphologic change of remote organs and the influence of index that correlated with oxygen free radical(OFR) damage,then the influence of remifentanil on above-mentioned correlated index.The aim of this study was to evaluate the protective effect of remifentanil on remote organs injury,and its possible mechanisms,in order to provide some empirical study information for illuminating mechanisms and basic reference data for its clinical application.Methods:24 male and healthy Wistar rats were randomly divided into 3 groups as follows:sham operation group(Group S),intestinal ischemia reperfusion group (Group I/R) and remifentanil group(Group R).Each group accepted heparin 1mg/kg in order to inducing anticoagulation all over the body.Every Group separated SMA from abdominal cavity and continuously accepted 0.9%Sodium Chloride 10ml/(kg·h) using microinjection pump.Group I/R clamped SMA for 1 h.Group R clamped SMA and received remifentanil whose volume equals to Sodium Chloride 3μg/(kg·h) intravenously 5 min before reperfusion.Two hours later,4 ml blood was gathered from inferior vena cava,centrifuged,and collected the supernatant fluid to measure the activity of ALT,AST,BUN and Scr.Cardiac lobe of right lung,caudate lobe of liver and cortical part of right kidney were conserved by liquid nitrogen to make homogenate for measuring the activity of NO,NOS,SOD and MDA.Lobus apicalis of right lung,internal lobe of left liver and left kidney were kept in 10% formaldehyde,then dyed by hematoxylin eosine(HE) and were observed the morphologic change.Data was carried on statistical analysis processing using the SPSS 16.0.Results:1.Pathological change of lung:we found that telangiectasis,hyperemia, manifold polymorphous neutrophils existed in interstitial tissue of lung,pulmonary atelectasis occurred in local areas and pulmonary alveolus edema and hemorrhage in Group I/R.The pathological change of Group R was slighter than Group I/R.2.The NO,NOS and MDA activity of lung homogenate were higher while the SOD activity was significantly lower in Group I/R than in Group S(p<0.05).Compared with Group I/R,the NO,NOS and MDA activity were lower while the SOD activity was higher in Group R(p<0.05).3.Pathological change of liver:we found that capacity of hepatic cells augmentation, engorgement,endochylema dyed shallow and hepatic sinusoid unclear in Group I/R. Hepatic cells only engorged in Group R.4.The ALT,AST and MDA activity of liver homogenate were higher while the SOD activity was lower in Group I/R than in Group S(p<0.05).The ALT,AST and MDA activity were lower while the SOD activity was higher in Group R comparing with Group I/R(p<0.05).5.Pathological change of kidney:we found that postglomerular capillary significantly dilated and manifold disruptive erythrocytes were in postglomerular capillary in Group I/R.Compared with Group S,the change of Group R was slighter.6.The BUN,Scr and MDA activity of kidney homogenate were higher while the SOD activity was lower in Group I/R than Group S(p<0.05).The BUN,Scr and MDA activity were lower while the SOD activity was higher in Group R comparing with Group I/R(p<0.05).Conclusion:1.We successfully set up the intestinal ischemia reperfusion rat model and observe that intestinal I/R could lead to injuries of remote organs.It induced the activity of antioxidase attenuation,the production of OFR increase,the degree of lipid peroxidation enhancement and leaded to changes of above-mentioned correlated index.2.Remifentanil could protect injuries of organs in rats inducing by intestinal I/R.It was possibly dued to improve the activities of antioxidase,clean the OFR,reduce the degree of lipid peroxidation,active opioid receptors and so on.
