[Objectives]Ecosystem of ocean is different with mainland.It has different pathogenic microorganism species and distribution.Pathogenic bacteria of open craniocerebral injuries with seawater immersion have its own character in species and distribution.The effect of irrigation treating infection with broad-spectrum antibiotics is not certain.This study is to analyze the kinds and distribution of pathogenic bacteria and investigate the therapeutic effect of irrigation of severe open craniocerebral injuries with seawater immersion.All open craniocerebral injuries with seawater immersion patients need debridement.Tradition opinion is to reserve brain tissue as much as possible,that is to maximatily decrease debridement range.But that method would be discussed under the seawater immersion environment.This study demonstrate different debridement range affecting life time of severe open craniocerebral injuries with seawater immersion.And hope that would be helpful to clinical rescue and treatment.[Materials and Methods]Weight-drop severe open craniocerebral injuries models of rabbits were established.Rabbits were random equally divided into three groups.Antibiotics irrigation group were irrigated with broad-spectrum antibiotics(1%Cefoperazone and 0.1%Levofloxacin).Physiological saline irrigation group were irrigated with physiological saline,and control group were not irrigated.Bacterial culture of brain surface were made when immersed 30 minutes and injured 24 hours. Observe the culture outcome and the living time in a week.Another a body of rabbits were divided randomly into three groups:large scale debridement group,small scale debridement group,and control group. Seawater immerse all the rabbits with severe open craniocerebral injuries.After eight hours,large and small scale debridement groups accepted debridements respectively,and control group didn't.Observe rabbits' life time in a week and whether colliquation or necrosis of brains happened.[Results]Weight-drop severe open craniocerebral injuries models of rabbits were established successfully.And the seawater immersed wound met the demand of experiment.There were significant difference of gram negative bacilli between antibiotics irrigation group and physiological saline irrigation group.Control group live much shorter than other two groups.Large scale debridement group lived significantly longer than small scale debridement group and control group, small scale debridement group lived significant longer than control group.There were no colliquation or necrosis in large scale debridement group,some in small scale debridement group,and much in control group.[Conclusion]The pathogenic bacteria of severe open craniocerebral injuries with seawater immersion are many kinds and species:gram negative bacilli are the major,and staphylococci are the second.Irrigating with broad-spectrum antibiotics can make rabbits live longer and reduce the possibility of intracranial infection.Large scale debridement can obviously prolong the life time of severe open craniocerebral injuries with seawater immersion after eight hours.
