Objective:To observe the impact of continuous infusion of remifentanil on cardiac muscle tissue cox-2 and serum H-FABP in dogs with myocardial ischemia-reperfusion injury.Methods 24 healthy hybrid canines were randomly divided into four groups(n=6):sham operated group(S),ischemia-reperfusion group(I/R),remifentanil groupⅠ(RⅠ) and remifentanil groupⅡ(RⅡ).The S group was not implement coronary artery occlusion.The I / R group was established by occluding the anterior descending branch of left coronary artery with 30min ischemia followed by a 120min reperfusion.The RⅠgroup underwent 0.5μg /(kg·min) microinjection pump intravenous infusion of remifentanil before ischemia,observation was carried out until the end of ischemia-reperfusion treatment same as on I / R group.In RⅡgroup, 1μg /(kg·min) microinjection pump intravenous infusion of remifentanil was performed before ischemia,observing until the end of ischemia-reperfusion treatment as on I / R group.The contents of H-FABP in serum were measured at four time points:30min pre-ischemia(T1), ischemic 30min(T2),reperfusion 60min(T3),and 120 min of reperfusion(T4).The expressions of cox-2 in the cardiac muscle tissue were measured at 120min after referfusion by immunohistochemistry.The serum H-FABP was valued using enzyme linked immunosorbent assay(ELISA).Results cox-2:Compared with the S group,the level of cox-2 in I/R group strengthened (P<0.05).The level of cox-2 in I/R group is higher than in RⅠand RⅡgroup(P<0.05).The level of cos-2 in RⅠgroup is higher than in RⅡgroup(P< 0.05).H-FABP:H-FABAP(ischemic 30min,reperfusion 60 min)in I/R group is higher than in S group(P<0.05).H-FABAP (ischemic 30min,reperfusion 60 min)in RⅡgroup is higher than in RⅠand I/R group(P<0.05). H-FABAP(ischemic 30min,reperfusion 60 min)in I/R group is higher than in RⅠgroup(P< 0.05).Conclusion Myocardial ischemia-reperfusion injury could increase the cardiac muscle tissue cox-2 and Serum H-FABP in dogs.Intraoperative continuous infusion of remifentanil could reduce the cardiac muscle tissue cox-2 expression in dogs with myocardial ischemia-reperfusion injury.Remifentanil involved in myocardial ischemia-reperfusion,with 0.5μg·kg-1·min-1,it could prevent myocardial cell injury;however with 1μg·kg-1·min-1,it could strengthen myocardial cell injury.
