[Objective] To study the correlation risk factors and the charactertics of different glycometabolism stages in impaired glucose regulation(IGR) and diabetes Mellitus(DM) in Guangxi district,and assess the value of different cut-point of fasting plasma glucose(FPG) for screening of DM and IGR.[Methods] A cross-sectional survey with multiple stage and random sampling was performed. The residents (aged 5 years old and over) were selected to do FPG and Oral glucose tolerance test(OGTT) in 5 city points (Nanning,Qinzhou,Luzhai,Hezhou,Baise and Hechi) in Guangxi from 2003 to 2005.The data was studied according to the 1999 diagnosis standards of WHO.The sensitivity,specificity,Youden′s index,diagnostic accuracy,positive predictive value,negative predictive value,positive and negative likelihood ratio of all the tests were evaluated by receiver operating characteristics curve(ROC curve).The optimal cut-point of FPG related to DM and impaired glucose tolerance(IGT) diagnosed by OGTT was determined.[Results]1.In Guangxi district,the average FPG were 5.14±1.42mmol/L, and the 95% Confidence Interval(CI) were 5.12~5.15mmol/L.The average FPG for male was 5.17±1.42mmol/L,and 5.09±1.42mmol/L for female.The average 2h postprandial plasma glucose(2hPG) was 6.87±2.93mmol/L,and the 95% CI were 6.84~6.91mmol/L.The average 2hPG for males was 6.98±2.99mmol/L, and 6.73±2.86mmol/L for female.There was no difference of the average FPG and 2hPG between male and female. The average of FPG and 2hPG increased with the age until reaching the top at the age of 60 years.2.The average of FPG and 2hPG in normal glucose tolerance (NGT) were 4.76±0.61mmol/L and 5.77±1.04mmol/L respectively,the 95% CI of FPG and 2hPG were 4.75~4.77mmol/L and 5.76~5.79mmol/L respectively.The average FPG of DM in female were higher than that in male. There was no difference of the average 2hPG of DM between male and female. The average FPG and 2hPG of DM had no correlation with the age.(r=-0.561, P=0.116;r=-0.535,P=0.138).3.HOMA-insulin resistance (HOMA-IR) was the highest factor among Impaired fasting glucose(IFG) + IGT groups and combined hyperglycemia(CH) groups(p<0.05).4. Using the ROC curve,the optimal cut-point of FPG related to IGT diagnosed by OGTT was 5.10mmol/L that was associated with a sensitivity and specificity of 62.84% and 61.99% respectively.Area under the curve was 0.651(P<0.000),a Youden′s index was 0.243.the optimal cut-point of FPG related to DM diagnosed by OGTT was 5.775mmol/L,which was associated with a sensitivity and specificity of 77.4% and 88.24% respectively.Area under the curve was 0.898(P<0.000,95%CI 0.889~0.907), a Youden′s index was 0.66,a diagnostic accuracy was 87.53%.[Conclusions]1.The average and the 95% CI of FPG were 5.14±1.42mmol /L and 5.12±5.15mmol/L respectively.There was no difference of the average FPG between male and female and the average of FPG increased with the age until reaching the top at the age of 60 years.2.The average of FPG and 2hPG in normal glucose tolerance (NGT) were 4.76±0.61mmol/L and 5.77±1.04mmol/L respectively,the 95% CI of FPG and 2hPG were 4.75~4.77mmol/L and 5.76~5.79mmol/L respectively;the average FPG and 2hPG of DM had no correlation with the age.3.The clinical metabolic disorder existed in the IGR state and was worst in IFG+ IGT groups,with most severe lipids disorder,Hyperinsulinemia and HOMA-IR.H OMA-IR andβ-cell function failure were obviously found in CH groups. Hyperinsulinemia existed in the isolated fasting hyperglycemia (IFH) state.4.Age,family history of DM,hyperlipidemia,hyperuricacidemia,hypertension, Central obesity and Hanzu were the major risk factors for type IGR.5.The value of FPG for screening IGT was low.6.The best cut-point of FPG was 5.775mmol/L for DM,which is eligible for screening of DM.
