The Meaning Of Each Time Point Plasma Glucose At OGTT In Diagnosis Of Abnormal Glucose Metabolism And The Characteristics Of Glucose Metabolism Of 1h Hyperglycemia

Objective To determine the cutoff value of plasma glucose at different time points during OGTT to diagnose diabetes and 1GR.And explore the relationship of 1h plasma glucose,glucose curve shape and islet B cell function,insulin sensitivity. Methods 4731 subjects were recruited to have an OGTT. The cutoff value of plasma glucose at different time points are analyzed by receiver operating characteristic curve (ROC curve).And based on the diagnosis standard of impaired glucose regulation and diabetes,we separate high 1h plasma glucose from the three groups by the 1h plasma glucose cutoff value.Then, we compare the level of plasma glucose and insulin,and analyze the correlation of 1h plasma glucose shape index and area under curve of glucose with HOMA-IR HOMA-B and insulingenic index.Results According to ROC results,to the diagnostic criteria for DM Omin,30min, 60min,180min plasma glucose cut point values are 5.93,10.79,12.94,7.77 mmol/L. area under-the curve of glucose (AUCg) cutoff value is 30.10 mmol.h/L,and the area under the ROC curve (AUC) are all greater than 90;to IGR for the diagnostic criteria for 0min,30min,60min,180min plasma glucose cut point values are 5.12,8.69,9.82,5.24mmol/L, AUCg cutoff value is 23.59mmol.h/L, and AUC of 0min,60min and AUCg are greater than 80.After grouping, compare each group of plasma glucose and insulin levels,they all have statistically significant difference, the insulin peak of 1h hyperglycemia group delay, and insulin secretion is significantly lower than 1h normal plasma glucose group.There was a negative correlation between 1hPG and insulingenic index,HOMA-B and a positive correlation between 1hPG and HOMA-IR.And also there is a negative correlation between AUCg and shape index with HOMA-B and insulingenic index and a positive correlation with HOMA-IR.Conclusions In addition to fasting plasma glucose and OGTT2hPG, OGTT other time points of plasma glucose can also provide us with lots of information. 1hPG islet B cells may reflect the function and insulin resistance,and high IhPG group compared with normal 1hPG, islet B cells dysfunction.In addition,the shape index, and AUCg can reflect the islet B cell function and insulin resistance.
