[Objective]: To assess the Arthroscopic Core Decompression and Bone Grafting in the management of avascular necrosis of femoral head.[Methods]: A retospective study was done in 60 patients(83 hips),from March,2002 to January,2008.who met the criteria of ficat stage(II).In which,18 cases(31 hips) in the modified surgery group were treated by Core Decompression combined with bone grafting on Arthroscopy;42 cases(52 hips)in the traditional group were treated by core decompression combined with bone grafting.Clinical evaluation was done based on HARRIS hip function scores,radiological evaluation by the classification system of FICAT and Alert.The post operative HARRIS score≥80 was defined as clinical success,and postoperative stable staging as radiological success. [Results] :The clinical success rate of the modified group and traditional group was 83.9% and 63.5% .respectively. The radiological success rate of the two groups was 80.6% and 61.5% .[Conclusion ] Arthroscopic Core Decompression combined with bone grafting can efficiently decrease intraosseous pressure and im prove the intraarticular environment, removal of necrotic bone Completely ,Effective pain relief and Improve joint function .The application of hip arthroscopy opens up a new way for the diagnosis and therapy of avascular necrosis of femoral head.This method is indicated for early stage(stageII)lesions.The difference of the Core Decompression combined with bone grafting on Arthroscopy and Core Decompression combined with bone grafting needs to be further investigated.
