Clinical Observation On Chronie Renal Failure And Experiment Study On Acenine-Induced Chronic Renal Failure Model In Rats With Shenqi Jiedu Decoction

Objective:glomerular sclerosis and interstitial fibrosis are the most important reason of the improvement of chronic renal failure,we observed the effect of Shenqi Jiedu decoction on chronic renal failure suffers and on rats of Adenine-induced chronic renal failure to approach the effects of the prevention and the mechanism of Shenqi Jiedu decoction.Methods:(1) clinical study:68 CRF cases were randomly divided into therapy group (treated with Shenqi Jiedu Decoction,general western medicine and losartan)(n=34) and controlled group(using general western medicine and losartan)(n=34).Each treatment period was 4 weeks and there were two periods.The changes of total effective rate,serum creatinine(SCr),blood urea nitrogen(BUN),creatinine clearancerate(Ccr),hemoglobin(Hb),plasmaalbumin(ALB),triglyceride(TG),cholestero l(CHO)were compared after treatment.(2) animal experimental study: Adenine-induced chronic renal failure model in rats was Established.All animals were randomly divided into six groups:the normal group,the model group,the high dose group,the medium dose group,the low dose group and losartan group,After 8 weeks of treatment,the levels of SCr,BUN,TG,CHO,ALB,Hb,nitric oxide(NO)and endothelin-1(ET-1) were tested in the six groups.In addition,the results of pathology and the expression of PDGF-B,PDGFR-βand CTGF were observed.Results:(1) clinical study:The total effective rate of the treating group was 88.24 percent,while the control group was 61.18 percent,The comparison of the curative effect in two groups were significant(P＜0.05).Two groups both could decrease SCr,BUN,and increase Ccr,Hb,ALB.The decrease of SCr,BUN,and increase of Ccr,Hb in the treated group was more significant than in the control group (P＜0.05).(2) experimental study:Compared with the model group,levels of SCr,BUN,CHOin the Shenqi Jiedu Decoction groups and the losartan group were decreased while the levels of ALB,Hb were increased(P＜0.05).ET-1 in the Shenqi Jiedu decoction groups and the losartan group were decreased compared with the model group while the levels of NO were increased(P＜0.05).Pathological results of Shenqi Jiedu Decoction groups and the losartan group were better than those of the model group,and of those the high dose group were the best.The gray scale of the immunohistochemistry staining indicated that the expression of PDGF-B,PDGFR-β,CTGF in the Shenqi Jiedu Decoction groups and the losartan group were decreased compared with the model group,and the high dose group is gradually descending.Conclusion:Shenqi Jiedu decoction can obviously improve the renal function in chronic renal failure suffers.With regard to the rats of Adenine-induced chronic renal failure,Shenqi Jiedu decoction can eficiently decrease the SCr,BUN,CHO levels and increase the ALB,Hb levels,increase the expression of NO and decrease the expression of ET-1,reduce the pathological injury and PDGF-B,PDGFR-β,CTGF expression.And we presume that the mechanisms of Shenqi Jiedu decoction delaying the chronic renal failure correlated with the relief of glomerular sclerosis and interstitial fibrosis.
