PrefaceInsulin resistance(IR) is the body of the biological effects of a certain amoun of Insulin to normal levels lower than expected,that insulin in promoting glucose uptakeand utilization of damage,excessive secretion of the body compensatory hyperinsulinemia of insulin produced to control the blood glucose levels.Current IR is considered an independent risk factor for cardiovascular disease,and hypertension, metabolic syndrome,type 2 diabetes,atherosclerosis,and many other diseases. Epidemiological studies found that many patients with essential hypertension associated with insulin resistance(IR),expressed as hyperinsulinemia and impaired glucose tolerance.Most scholars believe that IR is an independent risk factor for hypertension,the choice of antihypertensive drugs,not only have a satisfactory reduction in blood pressure,protecting the target organs,need to consider the impact of drugs on IR.Adiponectin(ADP) is secreted by fat cells,peptide hormones,and coronary heart disease,high blood pressure,insulin resistance,hyperlipidemia,type 2 diabetes and obesity are closely linked.Recent clinical studies have shown that application of angiotensinⅡreceptor blocker(ARB) step-down at the same time have increased serum adiponectin levels,improve insulin resistance.This study by observing the olmesartan treatment of essential hypertension and insulin resistance in patients with serum adiponectin concentration before and after the change and observe the level of blood pressure and IR to explore the new ARB drugs olmesartan pairs of adiponectin and IR effects.Experimental MethodsThe 45 patients with mild to moderate EH patients(to exclude secondary hypertension,severe liver and kidney dysfunction,coronary heart disease,stroke, diabetes,cancer,etc.) to give olmesartan 20mg qd,oral,treatment for 8 weeks during treatment to monitor changes in blood pressure before and after treatment of serum adiponectin(ADP),fasting blood glucose(FBG),fasting insulin(FINS),C-peptide(CP) level,calculated insulin sensitivity index(IRI).Another take the normal control group (45 persons) or more biochemical test as normal controls.EH patients were on medication for blood pressure monitoring during the period and record adverse reactions.To do two sets of pre-treatment indicators for general information and biochemical comparison between the two groups analysis of the indicators after treatment with the pre-treatment were analyzed.Data are mean±standard deviation of that measurement data statistically analyzed by t test;count data lines X~2 test to P<0.05 as statistically significant difference.ResultsTwo groups of individuals by sex,age,BMI,fasting blood glucose,no significant difference;essential hypertension patients with systolic blood pressure,diastolic blood pressure,CP,FINS,IRI,were higher than the normal control group,low levels of serum adiponectin in the control group,the difference has statistical significance.EH group after treatment SBP(135±6),DBP(84±3) was significantly lower than pre-treatment SBP(155±8),DBP(98±4) levels(P<0.05).EH group after treatment ADP(16.1±3.1) was significantly higher than pre-treatment ADP(10.9±3.1) levels (P<0.05),IRI from the pre-treatment(10.1±2.7) decreased(7.6±2.6),statistically significant differences between(P<0.05).Conclusion1.patients with essential hypertension there are different degrees of IR.2.ADP and the essential hypertension with IR correlation.3.olmesartan can be elevated in patients with essential hypertension serum ADP levels,increased insulin sensitivity in patients with essential hypertension.
