Objective: To explore the plasma adiponectin(APN)levels in patients with essential hypertension(EH) of Xinjiang KaZakans and the difference of plasma APN levels in patients with EH between KaZakans and Hans, to explore the relations between plasma APN levels in patients with EH of Xinjiang KaZakans and insulin resistance(IR), blood pressure, blood glucose, blood lipids and obesity.Methods: Totally 127 Xinjiang Kazakans and 131 Hans cases were selected for the subjects. They were divided into 4 groups according to their blood pressure: EH group of Kazakans (n=67), control group of Kazakans (n=60), EH group of Hans (n=67), control group of Hans (n=64). Measurment of blood pressure, height, weight, waist circumference(WC)and hip. Fasting venous blood was drawn to detect the plasma APN, fasting insulin(FINS), fasting plasma glucose(FPG), total cholesterol(TC), triglyceride(TG), high-density lipoprotein cholesterol(HDL-C), low-density lipoprotein cholesterol(LDL-C), Apoprotein-A1(Apo-A1) and Apoprotein-B(Apo-B),calculating body mass index (BMI), waist-hip ratio(WHR), body fat percentage(BF%) and Homeostasis model assessment of insulin resistance index(HOMA-IR). Plasma APN and insulin concentrations were detected by enzyme linked immunosorbent assay(ELISA) and radioimmunoassay(RIA) respectively. The data were analyzed with t-test,oneway analysis of variance, correlation and multiple stepwise linear regression analysis.Results: 1. The plasma APN concentrations in EH groups of KaZakans and Hans were significantly lower than those in control groups(P<0.01).2. There was no significantly difference of comparison of plasma APN level in EH groups or control groups between KaZakans and Hans(P>0.05).3. The plasma APN levels in EH groups of Kazakans and Hans were negatively and significantly correlated with systolic blood pressure(SBP),diastolic pressure(DBP),WC,BMI,WHR,FPG,FINS,HOMA-IR and TG(P<0.05 or 0.01). The plasma APN were also negatively and significantly correlated with LDL-C and Apo-B in EH groups of Kazakan(sP<0.05)and positively and significantly correlated with Apo-A1 in EH groups of Hans(P<0.01).We except the influential factors such as age, obesity, blood glucose and blood lipids, the APN levels in Kazakans and Hans have mainly negatively and significantly correlated with WC,BMI,WHR,FINS and HOMA-IR(P<0.05 or 0.01), and have no correlated with SBP and DBP(P>0.05).4. Multiple regression analysis showed that HOMA-IR and WC were independent factors to affect the plasma APN level in EH group of Kazakans,while HOMA-IR,WC and Apo-A1 were independent factors to affect the plasma APN level in EH group of Hans.5. The plasma APN levels in Class 1 EH patients both in Kazakans and Hans were significantly lower than those in the control groups(P<0.01). The plasma APN in Class 2 and 3 EH patients were significantly lower than those in Class 1 EH patients (P<0.05 or P<0.01).but the plasma APN in Class 2 and 3 EH patients have no significantly difference.6. The plasma APN levels in EH groups of women both in Kazakans and Hans were significantly higher than those in men(P<0.05 or 0.01).Conclusions: 1. The plasma APN level was significantly lower in patients with EH of Kazakans compared with the control group of Kazakans. 2. There was no significantly difference of plasma APN level in patients with EH between KaZakans and Hans.3. There were close correlations between plasma APN and IR or obesity in EH patients of Kazakans. APN may affect blood pressure, blood glucose and blood lipids by IR and obesity indirectly. HOMA-IR and WC were independent factors to affect the plasma APN level in EH group of Kazakans.
