Objective: To construct the model of rat kidney transplantation in order to observe the changes of kindey histological and kindey function on rat's ischemia-reperfusion(I/R)injury after kidney transplantation.To study the ischemia-reperfusion injury effects on kindey cell apoptotic index. To investigate effects of Danhong Injection on the expression of Bax. Bcl-2 and its protection effects on kindey histological and kindey function, to reveal the protection mechanism of Danhong injection on rat's ischemia-reperfusion injury after kidney transplantation.Methods: Ten normal SD rats were allocated into the sham operation group.And the rats were only been anesthetized and exposed kidneys for 45 minutes. The allogeneic male SD rats model of kidney transplantation were established。The recipients were divided into 3 groups (n=10,in each)at random :kidney transplant group, Danhong pretreatment group,Danhong treatment group. Observed kindey histological and inflammation cell infiltration changes through light microscope . Blood urea nitrogen (BUN) and creatinine( CR) levels were determined after 24 hours。The expression of Bax and Bcl-2 in kidney were quantitatively detected by immunohistochemistry.Cell apoptotic index was measured by TUNEL method.Results: 1. There were significant pathological changes in kindey tissue appearance in kidney transplant group ( the kidney tubular epithelial cells arranged disorderly,different degrees of necrosis,pyknosis and inflammatory cell infiltrations )compared with sham group.There are not obvious changes in Danhong treatment group Compared with kidney transplant group .Compared with those in kidney transplant group and Danhong treatment group , the kindey tubules epithelial cells swelled mildly and the inflammatory cell infiltration was obviously relieved in Danhong pretreatment group.2. Compared with those in the Sham group,the other groups BUN and CR increase to higher level(P <0.01). There are not obvious changes in Danhong treatment group Compared with kidney transplant group(P >0.05) .Compared with those in kidney transplant group and Danhong treatment group,BUN (45.16±6.50 mmol/L vs 69.96±6.22 mmol/L, P <0.01;45.16±6.50mmol/Lvs67.12±5.86mmol/L,P<0.01)CR(269.87±41.49 ummol/L vs 404.08±45.39 ummol/L, P <0.01;269.87±41.49 ummol/L vs 393.71±44.19 ummol/L, P <0.01) were decreased significantly.3.Compared with those in the Sham group,the cell apoptotic index (39.07%±2.57%vs5.17%±1.18%, P <0.01)and the expression of Bax and Bcl-2 (P <0.01)were significantly increased,and the Bcl-2/Bax ratio(P <0.01) was decreased significantly in the other groups . Compared with those in kidney transplant group and Danhong treatment group,the apoptotic index(21.18%±1.58%vs39.07%±2.57%, P <0.01; 21.18%±1.58 %vs38.24%±2.06%,P <0.01)and the expression of Bax were decreased significantly (P <0.01),the expression of Bcl-2(P<0.01)and the Bcl-2/Bax ratio (P<0.01) were increased significantly in Danhong pretreatment group.Conclusion:1. Kidney ischemia-reperfusion injury can cause major injury of nephridial tissue ,inhanced inflammation cell infiltration in the kindey tissue ,induced the aggravation of kindey function.2. Apoptosis participate in ischemical reperfusion injury, and Bcl-2,Bax gene also articipate in ischemical reperfusion injury. They decide apoptotic index by regulating Bcl-2/ Bax expression.3.Use Danhong injection before kindey transplantation can lessen the damage of kindey function and nephridial tissue after ischemical reperfusion injury , and can protect the kindey function and histomorphology . The intervention of Danhong injection can regulation the express of Bcl-2 and Bax to decrease apoptosis .
