In this paper, a system which consists of 3 layers which are target layer, criteria layer and index layer is builded. Target layer reflects the overall level of center, expressed by"Health examination center evaluation index". Criteria layer reflects different aspects (property) of center, including 6 aspects. Index layer contains 16 quantitative or qualitative indexs.In this paper, physical examination center is divided into four grades. And grade standard is developed for each specific evaluation index. According to the internal relationship betweent indexs and center, the weights of index system are determined by AHP. The most important factor in the criteria layer in relation to the target layer is the "hardware system", and its weight value is 0.3104; the largest share in the indicator layer in relation to the target layer is the "staffing", and its weight value is 0.1493.Thereafter, the paper uses the methods of fuzzy math analysis and system analysis to set up an AHP-fuzzy comprehensive evaluation model of the physical examination center. Then, four physical examination centers are evaluated by the model. The results show that the C11, C42, C53 are the common bottlenecks of the health examination center, and most of the health examination centers need focus on building the health management (B6).
