Objective:To observe the effects of hydrotalcite on histological quality of ulcer healing(QOUH) of active gastric ulcer(GU) patients with Helicobacter pylori(H.pylori) infection.Methods:145 active gastric ulcer patients with H.pylori infection were randomized into two groups:the control group were given esomeprazole alone after 7-days' anti-H.pylori triple therapy including Esomeprazole,Amoxicillin and Furazolidone,the treatment group was treated withthe same therapy plus hydrotalcite.After the fourth-week, both groups returned to visit to record their clinical symptom changes and ulcer healing rate which would be evaluated by gastroscopy.At the same time,two gastric biopsy specimens were collected from the same area near the ulcer as before.Then,the sections were stained with HE, AB-PAS,VG and SP inununohistochemistry method.The histological parameters that includ the morphological structure of mucosa,number of glandular organ,degree of inflammatory cell infiltration,number of microvessels,percentile of PAS positive area and collagen area, expression of epithelial growth factor receptor(EGFR) and basic fibrobast growth factor(bFGF) at ulcer margin regenerated mucosa were analysed by light microscope and computer image analysis techniques system(Normal group was 52 healthy gastric mucosa for SP inununohistochemistry method).Then 14 C-urea breath test(UBT) was rechecked 4 weeks after the end of the treatment for comparing the H.pylori eradication rate.Results:1.After treatments,there was no significant difference between these two groups in the clinical symptoms,H.pylori eradication rates and total ulcer healing rate which were evaluated by gastroscopy(P>0.05).2.After treatments,62.9%patients in the treatment group had excellent maturity at regenerated morphological structure of mucosa, which was 40.6%in the control group.There were significant differences between these two groups(P<0.05).3.After treatments,both groups had significant increase at the percentile of PAS positive area and collagen area of ulcer margin regenerated mucosa,and the treatment group had much more significant increase compared with the control group,there was significant difference between these two groups(P<0.05).4.The percentile of EGFR and bFGF positive cells at ulcer margin regenerated mucosa of the both two groups had significantly increased after treatments,and the treatment group were significantly higher than the control group(P<0.05).5.Comparing the H.pylori positive patients with the H.pylori nagative patients after treatments,there were no significant differences between these two groups in results of histological QOUH(P>0.05).Conclusions:1.The clinical symptoms,H.pylori eradication rates and total ulcer healing rate evaluated by gastroscopy of active gastric ulcer patients that using anti-H.pylori triple therapy,esomeprazole and hydrotalcite are of no significant difference with those used anti-H.pylori triple therapy and esomeprazole,but using anti-H.pylori triple therapy, esomeprazole and hydrotalcite can improve the histological QOUH.2.Hydrotalcite can improve the morphological maturity of healed gastric ulcer by optimizing morphological structure of mucosa,increasing number of glandular organ,decreasing degree of inflammatory cell infiltration,increasing number of microvessels and collagen area at ulcer margin regenerated mucosa.3.Hydrotalcite can improve the functional maturity of healed gastric ulcer by increasing neutrophilia mucus content,EGFR and bFGF expression at ulcer margin regenerated mucosa.
