The research of pathogenesis and treatment of ulcerative colitis has been delayed because of unknown etiology. In that case, the etiological factor directly or indirectly has become the key point of the research. With the industrialization of Aisa, the ratio of population suffered from ulcerative colitis increased year by year. The environmental factors attacted our attention as a possible reasson of ulcerative colitis. In the recent research about innate immunity of intestinal mucosa, people discovered a pattern recognition receptor, which can distinguish pathogeny from normal flora. Further more, ulcerative colitis is a process of chronic inflammation of the intestinal mucosa, which has a close relationship with flora imbalance. Intestinal flora shifts could be recognised as corpus alienum and lead to mucosal inflammation. And then, the mucosal is infiltrated major by leukomonocyte together with large number of eosinophile granulocyte. Eosinophile granulocyte is one of defense cells of innate immunity system and it take part in inflammatory reaction and antigen presentation.In this study we analysed eosinophile granulocyte infiltration of intestinal mucosa in patients with ulcerative colitis and studied the relationship between eosinophile granulocyte infiltration and other evaluating indicator such as CAI, EAI, histopathology index and trying to find something helpful for clinical treatment. Moreover, we studied the intestinal flora of both acute and remittent stage of patients with ulcerative colitis together with their spouse and first-degree relatives which could help us to understand the relationship of environment and transmissibihty affected to the disease. Also we detected the relationship between eosinophile granulocyte infiltration and flora imbalance.Methods:1. We choose patients diagnosed as ulcerative colitis according to the standard of ChengDu conference during December 2007 and November 2008 in the Second XiangYa Hospital.2. CAI, EAI and histopathology index were used to evaluate the activity of the diseases.3. Counting of eosinophile granulocyte: HE staining, count the number of eosinophile granulocyte on average of at least 5 high power field.4. Selective culture mediums were used for count the amount of bacteria. BS medium was used for Bifidobacteria, LBS medium was used for Lactobacillus, EMB medium was used for Escherichia coli, TTC medium was used for Enterococcus, hypersaline mannitol medium was used for Staphylococcus, BDS medium was used for Bacteroid and Rose Bengal medium was used for Yeast.Results:1. Disease activity coule be differentiated by eosinophile granulocyte infiltration in accord with CAI, EAI and histopathology index.2. The intestinal mucosa was infiltrated mainly by leukomonocyte together with large number of plasmacyte and eosinophile granulocyte, especially in active stage. Pathological changes of small vessels, glands and crypt abscess in lamina propia were more serious in active stage than remiting stage. Pathological changes in mesenchyma of remiting stage was slightly more than active stage.3. The counting of eosinophile granulocyte had no relationship with CAI, but negative correlation was found with EAI and histopathology index in active stage and positive correlation was found in remiting stage.4. Eosinophile granulocyte infiltration of intestinal mucosa had a close positive relationship with lymphadenosis and formation of lymph follicle while had negative relationship with focal hemorrhage, but had no relationship with other evaluating indicator in active stage patients. But in remiting stage patients, only the disappear of glblet cells had positive relationship with eosinophile granulocyte.5. Both in active stage and in released stage of ulcerative colitis patients, Lactobacillus decreased obviously compared with their first-degree relatives and healthy no relationship people while there's no difference among first-degree relatives and healthy no relationship people. Enterococcus in active stage was obviously higher than other groups while closely to the healthy relatives and no relationship people in released stage.6. There was no significance relationship between counting of eosinophile granulocyte and alternation of intestinal flora.Conclusions:1. Eosinophile granulocyte could be a possible indicator for evaluating active stage and remiting stage of ulcerative colitis.2. Intestinal flora disorder could be found in patients of ulcerative colitis. Lactobacillus decreased obviously both in active stage and released stage. In active stage, enterococcus also increased evidently.
