Protective Effects Of Hua Yu Ke Sai Fang Treatment On Acute Cerebral Infarction Of Syndrome Qi-insufficicncy And Blood Stasis And Its Mechanisms

ObjectiveTo explore the protective effects and its therapeutic mechanism of Hua Yu Ke Sai Fang (HYKSF) in treating the acute cerebral infarction by clinical investigation and animal experiment.MethodsSeventy patients with acute cerebral infarction hospitalized within 48 hours from onset were randomized to receive HYKSF treatment or conventional treatment. Fasting venous blood samples were collected on days 0,3,7 and 14 after hospitalization for detection of NSE and Hemorheology blood rheology. Meanwhile, the patients were evaluated using clinical neurological impairment scale at the same time spots. 80 rats were randomely divided into four groups (20/group):shasm-operated group,untreated control group,low dose (0.48/kg) Hua Yu Ke Sai Fang pretreatment group (treatment group 1) and high dose (0.96/kg) Hua Yu Ke Sai Fang pretreatment group (treatment group 2).Hua Yu Ke Sai Fang pretreatment group were treated by Hua Yu Ke Sai Fang 7 days before the model of middle cerebral artery occlusion, 1/day; whereas sham-operated group and untreated control group were given normal saline.On hours 24 and 72 after ischemia,rats were evaluated using Zea-Longa neurological impairment scale. Blood samples were collected from heart for detection of S100βat the same time. And then, rats were sacrificed by heads cutting and brains removing for the following studies: 1) Five rats in each group were used to estimate the volume of brain ischemic damage at each time object. The sections of the optic chiasma brain were stain infarct volume at the coronal level of the rostralin TTC, then the edge of the optic chiasm was calculated by image analysis; 2) Five rats in each group were used for HE stain at the same time. ResultsCompared with conventional treatment group, Hua Yu Ke Sai Fang can relieve obviously neurological deficit score, improve ability of daily life, relieve the score of syndrome qi-insufficicncy and blood stasis, decrease blood and plasma viscosity. Hua Yu Ke Sai Fang resulted in reduced serum NSE level also. On days 3 and 7, the serum levels of NSE in group HYKSF were lower than that in conventional treatment group, whereas the difference in serum NSE among the two groups on day 14 were significantly unremarkable.On experiment study, it made clear that Hua Yu Ke Sai Fang can decrease infarction areas, relieve neurological deficit score, reduce serum S100βlevel. Compared with sham-operated group, the neurological deficit and infarction volume of untreated control were increased significantly after ischemia injury. The percentage of apoptosis and intracellular calcium content were also obviously increased. Noticeable ischemic phenomenon was also seen in HE stain. Serum S100βlevel was increased significantly. In comparing with ischemia group, Hua Yu Ke Sai Fang can decrease the infarction areas, relieve neurological deficit score, reduce serum S100βlevel.ConclusionsThese results suggest that Hua Yu Ke Sai Fang can relieve obviously neurological deficit score, improve ability of daily life, relieve the score of syndrome qi-insufficicncy and blood stasis, decrease blood and plasma viscosity, reduced serum NSE level also. So in rats, Hua Yu Ke Sai Fang can protect the brain from ischemia injury, and the neuroprotections mechanism may be related to the decrease of NSE, S100β.
