Objective:In the research,analysed all patients' coronary arteriography checking outcomes and determined the urinary albumin(UAL) level and other clinical data.Ran interclass comparisons and multiple factor analysis.It is to investigate the features of coronary artery affection and the relationship between coronary artery affection degree and albuminuria and other facors in type 2 diabetic patients combined with coronary heart disease(CHD).Methods:From Nov.2007 to Oct.2008 in department of cardiology,Qilu Hospital,Shandong University,CHD patients were proved by coronary arteriongraphy.64 patients with type 2 diabetes mellitus in combination with CHD were enrolled in the diabetes group and 98 patients with simple CHD served as the control group.Patients of the diabetes group were subdivided into 3 groups based on scalar of the stenotic coronary artery, viz 1 ramus,2 rami and 3 rami affection groups.All patients didn't take ACE-I or ARB.All patients had no acute or chronic nephritis,pyelonephritis,desmosis,urinary tract infection,abnormal liver and renal function or tumor etc.Analysed all patients' coronary arteriography checking outcomes and determined the clinical data. Compared status of coronary artery affection and clinical data between the two groups and A/C among the 3 subsets in the diabetes group.In the diabetes group,age,course of DM,BMI,BP,BS,GHbAlc,blood-fat,A/C serving as iv,and affection sum and stenosis index serving as dv,ran polynary gradual regressive analysis.Results:1.Coronary arteriography outcomes analysis:Compared with the control group,the diabetes group had increased IN of poly-coronary artery affection,IN of diffuse affection,affection sum and stenosis index(P<0.05).2.Comparisons of the clinical data:Between the diabetes group and the control group, there were not statistical differences in age,hypertension,smoking,sex ratio,SBP, DBP,TC and LDL-C(P>0.05) and there were statistical differences in BMI,FBS,TG, HDL-C,A/C and albuminuria masc ratio(P<0.05).3.Comparisons of A/C among the three subsets in the diabetes group:3 rami affection group had increased A/C compared with 1 ramus and 2 rami affection group. 2 rami affection group had increased A/C compared with 1 ramus affection group (P<0.05).4.Multiple factor analysis:Affection sum had a colinearity with course of DM, P2hBS,GHbAlc and A/C(r=0.5773,0.4128,0.6281,0.5197,P<0.05).Stenosis index had a colinearity with SBP,LDL-C,HDL-C and A/C(r = 0.6077,0.4863,-0.3570, 0.5555,P<0.05)Conclutions:1.In type 2 diabetic patients in combination with CHD,coronary artery affection is extensive,diffuse and severe.2.Albuminuria is a risk factor for type 2 diabetic patients who will get coronary heart disease and A/C can reflex the degree of coronary artery affection to some extent.3.Long course of DM,postprandial hyperglycemia,increased GHbAlc,increased SBP,increased LDL-C and decreased HDL-C are other important risk factors for type 2 diabetic patients combined with CHD.
