The brain artery stenosis is the main reason for ischemic cerebral vascular disease. It is known that the brain artery system is composed by the anterior and posterior circulation. The posterior circulation (vertebral-basilar artery system) is more important, and its pathological change may bring in complicated symptoms. Vertebral-basilar artery stenosis leads to vertebrobasilar insufficiency (VBI) and its main symptom is vertigo.VBI is common in ischemic cerebral vascular disease.Because of its high rate of incidence and low rate of cure, if we don't give much attention to it, it can lead to acute cerebrovascular disease in the end. This disease belongs to vertigo syndrome in traditional Chinese medicine. The syndrome of blood stasis stagnant in orifice which is one of the most important syndromes of this disease is mainly study in this paper. Also it reviewed and summarized its etiology, pathology, treatment principle and treatment method. Based on these we probe the blood activating and orifice opening method is the main therapeutic policy to this syndrome. And it tries to probe into the functional mechanism of treatment method from the blood stream dynamics, blood rheology,etc.In this researeh, 70 patients with VBI were randomly divided into two groups.One group accepted the treatment of blood-activating and orifice-opening decoction, the other accepted Nimodipine as control. And it completes 33 cases of therapy group and 30 cases of control group. It tested some related parameters such as blood stream dynamics, blood rheology,and so on. Clinical results showed that total effective rate of therapy group was 96.97% and that of Nimodipine group was 83.33%. There was a significant difference between the two groups. In addition,it founds that the related parameters including TCD, blood stream dynamics, blood rheology, all had a delightful change, yet the treatment group had a better amelioration than the contrast group, and it showed a significant difference between the two groups(P<0.05,or P<0.01).The results indicated that the blood-activating and orifice-opening decoction is an effective method to this disease.It also puts porward ideas for continuous research on blood-activating and orifice-opening decoction in the future.
