Objective To investigate the clinicopathological features and prognostic factors of gastrointestinal stromal tumors.Methods The clinicopathological data of 327 patients with gastrointestinal stromal tumors admitted between January 1988 and Dec 2007 in Cancer Hospital of Chinese Academy of Medical Sciences was analyzed retrospectively using Statistical Package for the Social Sciences (Release 13.0, SPSS, Inc). Survival analysis was estimated using the Kaplan-Meier method and compared using the log-rank test. Cox regression was used in multivariate analysis.Results The 1 year, 3 years and 5 years total survival rate of 327 patients were 94.6%, 74.4% and 62.7% respectively. Flechers'classification , Sex, tumor size,mitotic rate, ,ki67 index, primary organ of tumor and resection margin were analyzed respectively . The differences were significiant ( P<0.05). Multivariate analysis showed that Flechers'classification ,ki67 index and radical surgical excision were the most important prognostic factor of the primary tumor resection group. 28 of 105 patients with advanced GIST patients received imatinib mesylate daily at dose of 400mg. Compared with other patients with advanced GIST,the median survial was 59 months and 30 months respectively. The differences were significiant ( P = 0.017).Conclusion Radical surgical excision is the best therapy to the primary gastrointestinal stomal tumors.Flechers'classification is rational and scientific. Imatinib mesylate is proved to be efective treatment of patients with advanced gastrointestinal stromal tumors.
