A Study On The Evaluation Of The New Rural Cooperative Medical System In Jiangsu Province

China has piloted the new rural cooperative medical system since 2003. The institutional framework and operating mechanism has established. The burden of medicine on the rural residents has been eased. The utilization health services have been improved. Poverty caused by diseases has been reduced. The basic medical institutions have developed. Meanwhile, the development of the new rural cooperative medical system is rapid in Jiangsu province, which is leading in the whole country. But the development is not balanced between counties in Jiangsu province. The comprehensive evaluation plays the important part in improving the the new rural cooperative medical system in Jiangsu province. Now the new rural cooperative medical system has transferred from pilot to comprehensive development, which provides the practical conditions for comprehensive evaluation of the new rural cooperative medical system in Jiangsu province.Firstly, this paper states the research background, the purpose and the significance. It summarizes the research literature at home and abroad about the evaluation of health insurance and new rural cooperative medical system. It also points out the contents, methods and research innovation of the study.Secondly, this paper defines the related concept and theories, such as the new rural cooperative medical system, comprehensive evaluation, indicators system, the stakeholder theory and system theory and so on.Thirdly, based on the combination of the particularity of the system and the needs of the practice work,using literature reading and expert consultation,this paper initially constructes the index system of the new rural cooperative medical system evaluation including three primary indexes,twelve secondary indexes and sixty-two three-level indexes. Then the Delphi method is used to choose the proper indicators. After two rounds of expert consultation, the final index system includes three primary indexes, eight secondary indexes and twenty-two three-level indexes.Then, the calculation of index weight chooses the the principal component analysis which is one of the objective weighting method. Then the indexes system has been applied in an empirical evaluation of the sixty-two counties in Jiangsu province and the ideal evaluation results have been achieved. The new rural cooperative medical system in Jiangsu province runs steadily in 2009. There are still some problems: the fund in some counties is overspending; medical costs in some counties have growth fast; actual hospital compensation in some counties is significantly low. To solve these problems, the following policy measures can be taken: the compensation scheme should be relatively unified and standardized with provincial cities for unites; automatic growth mechanism of financing standard should be established; some areas can have payment reform and further improve the reimbursement ratio; the management of the medical treatment out of counties should be strengthened; the information system upgrading should be continued for the early realization of network management of information system across the province. The empirical analysis also proves that evaluation index system is scientific and rational.Finally, this article gives several suggestions of construction the index system such as constructing the information system, unifying the compensation plan, regulating the system, choosing the statistical method and evaluation index rationally, supporting the evaluation of the new rural cooperative medical system. All these measures can promote the research of the new rural cooperative medical system in Jiangsu province.
