Background:In China, Budd-Chiari syndrome is mostly caused by inferior vena cava occlusion,while some by hepatic veins occlution. In this situation, we perform surgical treatment of Budd-Chiari syndrome by interposition graft caval-atrial shunt in proper patients, and obtained good treatment outcomes.Purpose:To evaluate the therapeutic short-term efficacy in surgical treatment of Budd-Chiari syndrome by interposition graft caval-atrial shunt.Methods:We reviewed 21 patients admitted to the department of hepatobilic pancreatic in Henan provincial people's hospital between May,2009 and March,2010. Patients were carried out with surgical treatment of Budd-Chiari syndrome by interposition graft caval-atrial shunt.5 of them are male, while 16 female. The average age is 40.6±7 years old (26-56). All patients were performed preoperatively evaluation of lower limbs and abdominal varices, and received electronic gastroscope inspection or upper gastrointestinal lyrics. We checked the liver function 3 days,6 days,9 days,14 days after operation. We checked lectronic gastroscope inspection or upper gastrointestinal lyrics in two weeks of postoperative. We checked DUS and liver function in 1 month and 2 month of postoperative.Results:In the group,20 cases of them could discharge from hospital in three weeks,1 case with lien chest drainage tube for large pleural effusion, and 3 months after discharge died of respiratory failure. The liver function of postoperative in 3 days,6 days was poorer-, transaminase and bilirubin are rising, with a statistical significance, but 9 days,14 days was worse than before without statistically significant. After two weeks, esophageal varices in 6 cases reduced.2 months later, interposition graft showed good, liver function was no worse before. Subjective evaluation of body varicose veins was better.Conclusions:Through the clinical observation, the application of interposition graft caval-atrial shunt in the patients with membranous obstruction or narrow of inferior vena cava can receive positive significance and prevent liver function of the patients from further deterioration, symptoms of inferior vena cava pressure hypertension can alleviate, such as varices of abdomen and lower limbs, can also get part catabatic of portal hypertension symptoms(esophageal varices), have short-term curative effect. We recommend a method for the operation. The advantage of this method is that he symptoms of patients improve obviously, but the disadvantages are trauma and artificial vascular long-term patent rate is not high.
