OBJECTIVE:1,To observe and assess the clinical effects of methycobal combined with lidocaine on primary trigeminal neuralgia.2, To compare the therapeutic effects of Methycobal combined with lidocaine and carbamazepine on primary trigeminal neuralgia.METHODS:To choose patients who suffered from PTN in Out-patient department of the hospital and then they were divided into two groups at random. In one group the patients were injected with mixed liquid of methycobal 1000ug and 2% lidocaine 2.0ml into bony aperture which nerves go through.30 cases in all.In another group the patients took carbamazepine tablets 100mg qd or bid, if no effect to add 100mg every day until to control hurt.regarding the minimum dose as optimal dose, as usual,600~1000mg every day.34 cases in all.The date were analyzed with visual analogue scale (VAS), The parameters included pain intensity before and 1 week,2 weeks,3 weeks,1 month, 2 months,3 months and 6 months after treatment.RESULTS: 1,there was no significant difference between two groups in gender, age and serious extent(P>0.05).2,In control group,24 cases had effect during first 6 months, about 70.6%.In treatment group,28 cases had effect during first 6months about 93.3%.Statistics showed there was a significant difference in the total effective rate between the two groups (P<0.05).The injecting treatment excels another one.3,Adverse reaction of treatment group is not appear, but there were five cases of drowsiness,3 cases of dizziness,1 case of nausea,1 case of dry mouth in the control group. There were not function change of the blood, urine and kidney in treatment period.CONCLUSION:Applications mecobalamine combined with lidocaine has obvious effect in treating primary trigeminal neuralgia, better than carbamazepine group. The adverse effect was little. It is valuable to research and extension.
