BackgroundLung cancer is a common fatal disease in the country, nearly 56 million people each year die from lung cancer, and the morbidity and mortality is rising, mainly due to smoking and environmental pollution. In all lung cancer cases,80% of the tissue types for non-small cell lung cancer (NSCLC). For early patients, surgical treatment of the expected 5-year survival rate was about 40% and 70% of patients failed in distant metastasis of cancer. Tumor growth and invasion caused distant metastasis, which leading to progression of the disease and patients with short survival. In recent years, the role of tumor angiogenesis in tumor development has received extensive attention. Tumor angiogenesis not only provide the necessary nutrients, and also provide opportunities of blood tumor cell proliferation. Study found that in many tumors, angiogenesis had close correlation to high tumor metastasis. Ang is a class of angiogenesis-related proteins, an early marker of Angiogenesis, which has close correlation to the number of tumor angiogenesis, clinical stage and prognosis.CD31, CD34, F-Ⅷ, and CD 105 are commonly used markers in a tumor vascular endothelial cells, which always be used for the study of microvessel density (MVD). These markers can quantitatively reflect the growth of tumor blood vessels, and indicate the trend of tumor growth, metastasis and recurrence.ObjectiveUsing CD105 and CD34 as markers of MVD, to detect the expression of Ang-2 (angiopoietin-2), CD105 and CD34 in NSCLC organization and the corresponding adjacent normal tissue, respectively. To study the correlation of Ang-2 expression levels in Lung Cancer withCD105 and CD34 marked MVD, and explore the relation between them and the infection to five-year survival rate of lung cancer. It will provide new ideas and theoretical basis for the diagnosis, treatment and prognosis of NSCLC.MethodsImmunohistochemistry SP method was used to detect the expression of Ang-2,CD105 and CD34 in 50 cases of NSCLC paraffin histopathological tissue (research group) and 10 cases of adjacent normal lung tissue (control group); Analyzing the differential expression of these factors, and exploring their relations to clinic opathological characteristics and disease progress survival. SPSS16.0 statistical software was used for statistical analysis.ResultsAng-2 expression in biopsy tissue was light yellow to brown granules, and mainly located in cytoplasm of vascular endothelial cells and the cell membrane matrix; CD 105 expression was located in cytoplasm and membrane of vascular endothelial; CD34 expression was brown granules, and located in vascular endothelial cytoplasm or cell membrane.1. In some tumor cells of non-small cell lung cancer tumors and microvessels, Ang-2 positive expression rate was 54%(27/50), and in adjacent normal lung tissues it was 20%(2/10). The difference was significant (P= 0.000). CD34 protein was both expressed in non-small cell lung cancer tissues and adjacent normal lung tissue capillaries and mature vessels. The difference of CD34-MVD in tumor tissue (17.53±9.32) and in normal tissues (10.33±3.44) was significant (P= 0.001). CD 105 protein was expressed in non-small cell lung cancer fringe tissue capillary. CD105-MVD in tumor was 12.64±4.64, and was weakly positive or negative in normal tissues (4.16±0.83). The difference was significant (P= 0.000).2. Ang-2, CD105-MVD and CD34-MVD expression had no significant correlation with age, sex, histological type and smoking status (P> 0.05); Ang-2, CD105-MVD and CD34-MVD expression were closely related to cell differentiation, tumor size, lymph node metastasis and clinical stage (P<0.05).3. CD105-MVD had higher expression in Ang-2 positive expression group (13.21±4.79). Compared with the negative group (7.07±2.71), and the difference was significant(P= 0.029). CD34-MVD had higher expression in Ang-2 positive group (25.68±6.65), and in the negative group it was 14.68±0.76. The difference was significant (P= 0.010). The correlation coefficient of Ang-2 expression with CD105-MVD expression was 0.632,P=0.001; and with CD34-MVD expression it was 0.201, P=0.043; indicating the expression of Ang-2 was positively correlated with MVD.4. The five-year cumulative survival rate in Ang-2 negative expression group was 47.83%(11/23), and in positive group it was 25.93%(7/27). Log-rank test:9.156. The difference was statistically significant (P= 0.002). The median survival of Ang-2 positive group was 35 months, and lower than the negative group with 51 months survival (P<0.05). The survival curves of Ang-2 positive expression group were significantly lower than those with negative expression. The five-year cumulative survival rate in CD105-MVD high expression group was 29.41%, and in low expression group it was 37.50%. Log-rank test:4.446, and the difference was statistically significant (P= 0.035). The median survival of CD105-MVD high expression group was 36 months, and lower than the low expression group with 56 months survival (P<0.05). The five-year cumulative survival rate in CD34-MVD high expression group was 27.78%, and it was 40.62% in low expression group. Log-rank test:5.393, and the difference was statistically significant (P= 0.020). The median survival of CD34-MVD high expression group was 37 months, and lower than the low expression group with 57 months survival (P<0.05). The survival curves of MVD high expression group was significantly lower than those with low expression. Conclusion:1,Ang-2 protein,CD105-MVD and CD34-MVD expression in lung cancer was significantly higher than that in adjacent normal tissue. The results showed that had relation to tumor occurrence and development of blood vessels.2,The expression of CD105-MVD and CD34-MVD in lung cancer with positive Ang-2 protein expression was significantly higher than that with negative Ang-2 expression,the results showed that Ang-2 positive expression was positively correlated with the expression of MVD.3,Ang-2, CD105-MVD and CD34-MVD expression in NSCLC were closely related to survival curves, and higher expression indicated shorter survival time.
