Changes And Significance Of Serum HIF-1α,OPN,sICAM-1,MMP-2 In Primary Hepatocellular Carcinoma Patients Before And After Transcatheter Arterial Chemoenbolization

Objective:To explore the impact of transhepatic artery chemoembolization(TACE) on hepatocarcinoma's invasion and attack through measuring the dynamic changes of serum hypoxia inducible factor-lα(HIF-lα),osteopontin (OPN),serum intercellular adhesion molecule (sICAM-1),matrixmetalloproteinase-2(MMP-2) in patients with primary hepatocellular carcinoma(PHC) before and after the treatment of TACE.Methods:The serum levels of HIF-1α,OPN,MMP-2 andsICAM-1 in 35 cases with PHC 1day before TACE, on the 2nd day and 30 day after TACE were measured by ELISA; Before the treatment, all of the patients received CT inspection,recording the number,diameter of the tumour and whether there is tumour thrombi or metastasis,and 30days after TACE we observed the changes of the tumor by CT or DSA.15 cases of healthy persons as a healthy control group.Result:Compared with control group,all of the serum HIF-1αOPN,MMP-2,sICAM-1 levels in patients with PHC before TACE increased significantly(all were P < 0.01); And the levels of the above-mentioned four indicators in the patients with tumor thrombi group were significantly higher than that in the patients without tumor thrombi group (all were P<0.05); The levels in patients with metastasis group significantly higher than that in the patients without metastasis groups (all were P<0.05).On the 2nd day after TACE,the levels of serum HIF-la and OPN were higher than that before the treatment(both P<0.05),while the levels of MMP-2 sICAM-1 were declined than that before TACE; One month later, compared with before and 2nd day after TACE,both levels of serum HIF-la and OPN significantly declined (both P<0.05),and the level of MMP-2 sICAM-1 increased to the levels of before TACE. In adition there were significantly correlation between HIF-la and OPN in patients with HCC (r=0.983, P<0.05) On the 30days after TACE,there were 16 patients took CT examination and 28 paitients had DSA when received TACE treatment,compared with before the treatment of TACE,the serum expression of HIF-1α,OPN,sICAM-1,MMP-2 in the tumour volume iodine oil deposit completenessly group reduced obviously.Conclusion:Relatively compared with before TACE, the expression of HIF-la and OPN increased on the second day after the operation, declined after 30days, and the expression of both has some connection, showing that TACE aggravates exhausted oxygen microenvironment of tumour on the 2nd day after the operation, makes the degradation of HIF-1αreduce, increases the secretion of OPN. With the tumor tissue's massive necroses, to oxygen demand reduction and the setting-up of the side circulation, it has relieved the tumour's oxygen deficiency. So the two expression dropped after 30 days. Relatively compared with before TACE, the density of sICAM-1 and MMP-2 declined on the second day after the operation, increased after 30days, showing that TACE can kill the cancer cell,make the expression of sICAM-1 and MMP-2 declined, With the tumour tissue's adaptation to short of oxygen, and the setting-up of the side circulation, it has relieved the tumour's oxygen deficiency. So the two expression increased after 30 days. But the level still not surpassed before the treatment, which prompt the treatsment has not increased the invasion and attack of the tumour.In a word,the dynamic changes of HIF-1α, OPN, sICAM-1, MMP-2 before and after TACE prove that it doesn't increase the invasion and attack of tumour while treating it.
