Study On Relationship Between Change Of Hepatic Pathology And Level Of Serum ALT, AFP, HbeAg, HBV DNA In Chronic HBV Carriers

[Objective] To study the relationships among the change of hepatic pathology and the level of serum ALT,AFP,HbeAg,HBV DNA in chronic HBV carriers.[Methods] Hepatic pathological examinations.liver function tests, serum AFP,HbeAg,HBV DNA detection were performed in 132 patients with chronic HBV carriers.[Results] (1)Thirteen patients (9.8%) had no change of inflammation,one hundred and eight patients(81.8%)had mild change of inflammation, ten patients(7.6%)had moderate change of inflammation, one patient(0.76%)had severe change of inflammation. (2)There were significant age differences among three groups(P< 0.05). (3)There were significant differences in Alanine transaminase levels among patients who had normal liver histology,mild liver histology and moderate histology(P< 0.05).(4)The AFP levels of normal liver histology are(5.27±5.59)ng/ml, the AFP levels of mild liver histology are(7.03±4.60)ng/ml, the AFP levels of moderate liver histology are(12.64±1.88)ng/ml. There were no significant differences in AFP levels between patients who had normal liver histology and mild liver histology(t=0.88,P> 0.05).There were significant differences in AFP levels between patients who had normal liver histology and moderate histology(t=2.36,P< 0.05).There were significant differences in AFP levels between patients who had mild liver histology and moderate histology (t=3.05,P<0.01).(5)There were no significant differences between the serum HbeAg positive group and the negative group(P> 0.05).(6)There were no significant differences between change of hepatic pathology and the level of serum HBV DNA(P> 0.05).[Conclusions] Most chronic HBV carriers had mild liver histology. The change of hepatic pathology has no correlation with the level of HBV DNA or the state of HbeAg positivity.The increased of age,ALT,AFP might be related to the change of hepatic pathology.Liver biopsy in cases of chronic HBV carriers is most helpful to assess accurately both the current activity of the disease and the degree of fibrosis and to estimate if antiviral therapy should be conducted.
