Objective:To explore the clinical result of using proximal femoral nail(PFN) internal fixation cure intertrochanteric fracture. Discuss its attentions, intraoperative and postoperative complications。Method:Retrospective analysis 91 cases from February 2007 to March 2010, using PFN curing intertrochanteric fracture. Male 55 cases, female 36 cases, age 40~100, 73.3 in average. According Evans classification standard,Ⅰ:7cases,Ⅱ:13 cases, IIIA27 cases,ⅢB21cases,Ⅳ17cases, reverse intertrochanteric fracture 6cases. Follow up their postoperative complications, healing time and function score.Result: Follow up 72 cases, from 6 to 24 months,18 months in average, all union.According Harris hip function classification standard, excellent 64 cases, 88.9%, fine 3 cases,4.2%, inferior 5 cases,6.9%. Intraoperative distal femur fracture 1 case, using femur interlocking intramedullary nail fixation; coxa adducta 2 cases; postoperative lag screw or anti-rotation screw cut whirlbone 2 cases; near-end lag screw drop out 6 cases; heterotopic ossification lcase. No broken nail cases.Conculsion:1,PFN curing intertrochanteric fracture has a well clinical effect, and is a ideal method of curing intertrochanteric fracture; 2,Good reduction of the fracture is a condition of the successful opreation; 3 The best position of the proximal nail is the lower 2/3 region in neck of the femur and fewer complications such cases; 4,Do not advocate using PFN for shot collum femoris and smaller diameter; 5,It is unnecessary taking out internal fixation after bone union.
