Objectives:To contrast the safety, clinical therapeutic effect and postoperative complications of ABO,DHS,PFN to treat in femoral intertrochanteric fracture, in order to provide clinical evidence of select the method in internal fixation.Methodes:From January 2006 to December 2009, we had operated 121 patients of A23 and A33 ICFF, in those patients, we internal fixed 44 cases of ABO,41 cases of DHS and 36 cases of PFN. Analyzed the age, length of stay, operative incision, operative time, volume of blood transfusion, wound drainage. However, we followed up the X-ray of postoperation 1 week,1 monthes,3 monthes,6 monthes,12 monthes, evaluated the union of fracture. Recorded the Harris score to evaluate the function of hip. Statistically analyzed the data with SPSS 17.0 edition software, test standardtion was P= 0.05.Results:1,The age in the ABO, DHS and PFN groups had no difference (P>0.05). The length of stay in PFN groups was short than ABO and DHS groups, in group of ABO and DHS, there were no difference.2,The length of operative incision, volume of blood transfusion and wound drainage in PFN groups was less than ABO and DHS groups, while the operative time in PFN groups was longer than them, the diference was statistically (P<0.05). In groups of ABO and DHS, there were no difference.3,The average time of fracture union in those three groups were 3.58±0.24 months,3.31±0.16 months and 3.15±0.25 months respectively, there were no difference. The were no disunion in those three groups. There were to cases of malunion in ABO group and none cases of malunion in DHS and PFN groups. The rate of malunion in DHS and PFN groups were lower than ABO group(P<0.05). 4,The Harris score in those three groups after 1 week had no difference. The score in PFN groups was higher than ABO and DHS groups after 1 month and 3 monthes, there were no statistical difference in the groups of ABO and DHS. There were no statistical diference of Harris score in those three groups after 6 monthes and 12 monthes (P>0.05)Conclusions:1,The average time of fracture union in ABO, DHS and PFN groups were short and Harris score were fine, there were an excellent method to treat ICFF at all.2,The length of operative incision, volume of blood transfusion and wound drainage in PFN groups were less than ABO and DHS groups, suggest that the surgical trauma in PFN is less.3,The average length of stay in PFN groups was short, and the Harris score in early time post operation is higher than ABO and DHS groups, advise that the interal fixtion stability in PFN is higher and the habilitation is quicker.4,The operative time in PFN groups was longer than ABO and DHS groups, suggest that the surgical fixation in PFN is harder and should practice by an experienced surgery.5,We should select a perfect method of internal fixation in ABO, DHS and PFN to treat a complicated femoral intertrochanteric fracture.
